What you should know about vaping and e-cigarettes

    I often hear from young parents

    who are new the game
    of worrying about their children.

    I tell them that it doesn't.

    It gets different,

    but there's always something
    to worry about as parents.

    I remember how I would lie
    half-awake at night,

    listening to my son breathe
    when he was young and had asthma.

    And then when he was a teen,
  • 0:26 - 0:30
    until I heard the front door chime open
    and I knew that he was home safe.

    Worrying about our kids
    comes with the territory.
  • 0:34 - 0:38
    Now, many of these worries
    are about basic issues
  • 0:38 - 0:39
    like what they eat,
  • 0:39 - 0:40
    where they are,
  • 0:40 - 0:42
    who they're with,
  • 0:42 - 0:45
    but we also need to keep an eye
    on new behaviors and fads.

    The latest craze is something
    that may not be yet on everyone's radar
  • 0:51 - 0:53
    as a serious health concern,
  • 0:53 - 0:56
    and that is the newfound
    popularity of vaping,
  • 0:56 - 0:58
    or inhaling sweet aerosols
  • 0:58 - 1:01
    produced by vaporizing
    e-liquids in e-cigarettes.
  • 1:03 - 1:04
    E-cigarettes,
  • 1:04 - 1:06
    or vapes, as they are commonly called,
  • 1:06 - 1:08
    are flying off the shelves like candy.
  • 1:08 - 1:09
    This year,
  • 1:09 - 1:13
    the e-cigarette market is expected
    to drive 26 billion dollars
  • 1:13 - 1:14
    in sales worldwide.
  • 1:15 - 1:16
    Over the next six years,
  • 1:16 - 1:18
    that volume is expected to double.
  • 1:20 - 1:23
    We have a lot of serious concerns
    about the health impact of vaping,
  • 1:23 - 1:26
    and unfortunately not nearly
    enough answers.
  • 1:27 - 1:31
    This becomes even more concerning
    when you think of who uses e-cigarettes.
  • 1:32 - 1:34
    E-cigarette use, at least in the US,
  • 1:34 - 1:37
    has grown rapidly among youth
    and young adults.
  • 1:38 - 1:39
    Our kids --
  • 1:39 - 1:41
    our most vulnerable population.
  • 1:42 - 1:46
    There was a 900 percent growth
    in the use of e-cigarettes by youth
  • 1:46 - 1:49
    between 2012 and 2015.
  • 1:50 - 1:52
    The most recent estimates suggest
  • 1:52 - 1:56
    that approximately 3.6 million
    high school and middle school students
  • 1:56 - 1:58
    have used e-cigarettes in the US.
  • 2:00 - 2:03
    Now, e-cigarettes were originally created
  • 2:03 - 2:05
    to offer smokers a cleaner
    form of nicotine
  • 2:05 - 2:07
    to help with their cigarette addiction.
  • 2:08 - 2:12
    In the US, these devices
    come under the FDA's jurisdiction
  • 2:12 - 2:13
    as a tobacco product,
  • 2:13 - 2:16
    but the science on these devices
    has not been able to keep up
  • 2:16 - 2:19
    with the rapid market growth,
  • 2:19 - 2:24
    so regulations on the components of these
    devices and e-liquids are lagging.
  • 2:26 - 2:29
    Current regulations do restrict
    sales of these devices
  • 2:29 - 2:32
    to anyone under the age of 18,
  • 2:32 - 2:35
    but these do not seem
    to have had much of an impact
  • 2:35 - 2:38
    on the explosion in the use
    of these devices by teens.

    You know the first time
    I heard and saw an e-cigarette,
  • 2:42 - 2:45
    I knew right away
    that teens would love it.
  • 2:45 - 2:48
    These devices are technology on a stick.
  • 2:48 - 2:51
    A perfect fit for
    the smartphone generation --
  • 2:51 - 2:58
    small, rechargeable, easy to use,
    easy to modify, nice smelling --
  • 2:58 - 3:01
    some even sync with you smartphone
    to let you know how much you have vaped.
  • 3:01 - 3:04
    Even I was very drawn
    to these very clever devices.
  • 3:06 - 3:10
    And since I had spent a long time
    researching teen and adult addictions,
  • 3:10 - 3:15
    I immediately realized that these devices
    fit perfectly into the teen psyche.
  • 3:17 - 3:20
    Teens are impulsive
    and they love to try new things.
  • 3:21 - 3:24
    They're also craving independence
    and they love to make things their own.
  • 3:25 - 3:28
    E-cigarettes meet these needs perfectly
  • 3:28 - 3:29
    by allowing them the chance
  • 3:29 - 3:33
    to both innovate and personalize
    their vape experience.
  • 3:34 - 3:39
    They can choose from over 15,000
    different e-liquid flavors
  • 3:39 - 3:41
    and multiple nicotine concentrations.
  • 3:41 - 3:45
    They can even create their own
    nicotine flavor combination.
  • 3:46 - 3:50
    They can change how much vapor
    is produced from these devices
  • 3:50 - 3:53
    by modifying the puff volume
    and the constituents
  • 3:53 - 3:55
    and the power and temperature
    of the devices.
  • 3:56 - 3:59
    They can even use these devices
    for cloud chasing.
  • 4:01 - 4:01
    Cloud chasing,
  • 4:01 - 4:05
    all called vape tricks or smoke tricks,
  • 4:05 - 4:09
    involves producing large vape clouds
    with quirky shapes and names,
  • 4:09 - 4:12
    like rings, dragons, coasts --
  • 4:12 - 4:15
    cloud chasers can even participate
    in cloud competitions
  • 4:15 - 4:18
    and win prizes for creating
    the most innovative shaped clouds.
  • 4:20 - 4:24
    Teens can also change the strength
    and throat hit from the vapor
  • 4:24 - 4:27
    by either vaporizing the e-liquid
    at higher temperatures
  • 4:27 - 4:30
    or dripping the e-liquid
    directly onto a heated coil.

    They can even use these devices
    for marijuana vaping.
  • 4:35 - 4:38
    And since the devices use
    lower temperatures
  • 4:38 - 4:40
    and do not combust or burn the marijuana,
  • 4:40 - 4:42
    they can do this very discreetly
  • 4:42 - 4:45
    without the distinctive smell
    of burnt marijuana.

    So they can really make these vape
    experiences their own,
  • 4:49 - 4:53
    which may explain the astounding rise
    in the use of these devices by youth.

    Now e-cigarettes are technically
    a very simple device.

    There is a receptable for the e-liquid
    which can be a tank, a pod or a plug,
  • 5:04 - 5:09
    there is a battery which charges the coil
    which then vaporizes the e-liquid,
  • 5:09 - 5:10
    and then there is mouthpiece
  • 5:10 - 5:13
    where the user can actually draw
    from the e-cigarette.
  • 5:14 - 5:19
    In 2017, there were 466 e-cigarette
    devices in the marketplace.

    These range from cigarette light devices
    which are also called cigalights
  • 5:25 - 5:29
    to tank systems
    which are also called pens
  • 5:29 - 5:33
    and then there are modified
    devices which are also called mods.
  • 5:33 - 5:39
    Now mods look nothing like a cigarette
    and they come in various shapes and sizes
  • 5:39 - 5:42
    with different kinds of attachments
    and user adjustments.
  • 5:42 - 5:44
    They're very popular for cloud chasing.
  • 5:45 - 5:47
    The most recent entrance
    into the marketplace
  • 5:47 - 5:49
    are the pod devices,
  • 5:49 - 5:52
    which contain the e-liquid in a pod --
  • 5:52 - 5:54
    these are very popular
    by the way among teens --
  • 5:54 - 5:57
    and an example of this is the Juul
  • 5:57 - 5:59
    which not only looks like a USB device
  • 5:59 - 6:02
    but can also be plugged
    into a USB outlet to charge.
  • 6:03 - 6:06
    Many teens do not even think
    that these are e-cigarettes
  • 6:06 - 6:10
    which has led to the use of terms
    like "Juuling" instead of vaping.
  • 6:11 - 6:15
    Many of these devices are so discreet
    and produce so little vapor
  • 6:15 - 6:17
    that teens are using them in classrooms
  • 6:17 - 6:21
    and hiding them in objects
    like Sharpie pens, their clothes,
  • 6:21 - 6:22
    their books.
  • 6:23 - 6:25
    Now many teens think that these devices
    produce water vapor
  • 6:25 - 6:28
    and therefore are safe to use,
  • 6:28 - 6:30
    but this could not
    be further from the truth.
  • 6:31 - 6:33
    What is produced is not even a vapor,
  • 6:33 - 6:34
    it's an aerosol,
  • 6:34 - 6:36
    and let me tell you, the difference
    is quite pronounced.
  • 6:37 - 6:41
    Aerosols contain many finely suspended
    particles of liquids and gases
  • 6:41 - 6:44
    that are created
    from whatever is in the e-liquid.

    So an aerosol could contain
    propylene glycol and glycerin,

    which are solvents in the e-liquid.
  • Not Synced
    Now these solvents are known
    to be safe for edible use,
  • Not Synced
    so for use in products that you eat,
  • Not Synced
    but we know very little about their safety
    following long-term inhalational exposure.

    The e-liquids can also contain alcohol,
  • Not Synced
    sometimes at high levels.
  • Not Synced
    And inhaling alcohol is known
    to have toxic effects on the brain.
  • Not Synced
    I told you earlier that the e-liquids
    contain over 15,000 different flavors.

    Here are some examples.
  • Not Synced
    Some with very catchy but familiar names
    like Skittles and Fruit Loops,
  • Not Synced
    and others with more exotic names
    like "Dragon's Milk" and "Tiger's Blood"
  • Not Synced
    and "Unicorn Puke."
  • Not Synced
    The e-liquid or the aerosol
    can also contain metallic particles
  • Not Synced
    like chromium, cadmium and lead.
  • Not Synced
    Now these are generated
    from the heating coil in the devices
  • Not Synced
    and are also known to have many toxic
    effects on vital organs.
  • Not Synced
    So no, let me make this very clear:
  • Not Synced
    what is produced is definitely
    not water vapor.
  • Not Synced
    Exposure of the teen brain to nicotine
  • Not Synced
    through e-cigarettes
    is also very concerning.
  • Not Synced
    The teen brain is very sensitive
    to even low levels of nicotine
  • Not Synced
    and gets very easily addicted.
  • Not Synced
    In fact, we have known for a long time
  • Not Synced
    that 90 percent of smokers start smoking
    cigarettes prior to the age of 18.
  • Not Synced
    Those who start early are more addicted
    and have a harder time quitting smoking.
  • Not Synced
    In other words,
  • Not Synced
    and to quote a past FDA commissioner,
  • Not Synced
    nicotine addiction is a pediatric disease.
  • Not Synced
    Now, e-cigarettes can expose teens
    to a lot of nicotine.
  • Not Synced
    Many of these devices contain
    the amount of nicotine
  • Not Synced
    that is in a full pack of cigarettes.
  • Not Synced
    The more recent port devices
    contain a nicotine salt,
  • Not Synced
    which has a smoother taste
    and is much easier to use
  • Not Synced
    and can produce rapid increases
    in brain nicotine levels.
  • Not Synced
    Teens who use e-cigarettes regularly
    report symptoms of craving,
  • Not Synced
    feeling anxious when they don't
    have their e-cigarettes.
  • Not Synced
    All these are hallmarks
    of behavioral addictions.
  • Not Synced
    Now, e-cigarettes are not only addictive
  • Not Synced
    but they also affect many other
    organs in the body.
  • Not Synced
    So nicotine, which is in e-cigarettes,
  • Not Synced
    for example,
  • Not Synced
    binds to a receptor called
    the nicotinic acetylcholine receptor,
  • Not Synced
    which plays a key role in the functioning
    of almost all organ systems
  • Not Synced
    in the human body.
  • Not Synced
    And chronic exposure to nicotine
    changes the functioning of these systems.
  • Not Synced
    So as an example,
  • Not Synced
    chronic exposure to nicotine decreases
    the flexibility of the blood vessels
  • Not Synced
    and changes how the heart responds
    to acute challenges like stress.
  • Not Synced
    The teen brain is not only sensitive
    to the addictive effects of nicotine
  • Not Synced
    but also to its toxic effects.
  • Not Synced
    In adolescent animals, nicotine
    is a very well established neurotoxin
  • Not Synced
    and it decreases learning,
    memory and attention processes
  • Not Synced
    and increases hyperactivity symptoms.
  • Not Synced
    Teens who use tobacco products
    are more likely to use marijauna
  • Not Synced
    and alcohol
  • Not Synced
    and also develop depression and anxiety
    as a teen or as an adult.

    So nicotine addiction through e-cigarettes
    could be leading them down the path
  • Not Synced
    of other addictions
    and other mental health problems.

    Now, in adolescent animals, nicotine
    also produces epigenetic changes,
  • Not Synced
    or heritable changes in gene expression.
  • Not Synced
    For example, in genes involved in asthma.
  • Not Synced
    So teens who use nicotine may not
    only be harming themselves
  • Not Synced
    but they could be harming
    their future generations.
  • Not Synced
    You know the very existence
    of e-cigarettes
  • Not Synced
    could have led to an entire generation
    of nicotine-addicted youth.
  • Not Synced
    Easy access to these devices
    could have led to more experimentation
  • Not Synced
    with marijuana and many other
    vaporizable substances by youth.
  • Not Synced
    While there is no doubt that providing
    smokers with a cleaner form of nicotine
  • Not Synced
    is and should continue to be
    a critical goal,
  • Not Synced
    we still do not know if these devices
    help smokers quit smoking,
  • Not Synced
    and we know very little
  • Not Synced
    about the long-term effects
    of these devices.
  • Not Synced
    What we do know is that youth --
  • Not Synced
    lots of youth are using these devices.
  • Not Synced
    In fact, the FDA commissioner
    recently used the term "epidemic"
  • Not Synced
    to describe e-cigarette use in the US.
  • Not Synced
    While trying to solve
    one huge public health problem --
  • Not Synced
    cigarette smoking --
  • Not Synced
    we may have created another colossal one.
  • Not Synced
    Our lack of vigilence in the earlier years
    around cigarette smoking
  • Not Synced
    led to a cigarette epidemic and many,
    many cigarette-related diseases.
  • Not Synced
    We do not want to repeat
    the same mistakes with e-cigarettes.
  • Not Synced
    So now is the time for action --
  • Not Synced
    for regulations that address the appeal
    and access of these devices to youth.
  • Not Synced
    Do smokers really need 15,000
    kid-friendly flavors to quit smoking?
  • Not Synced
    Do they need so many different
    kinds of devices?
  • Not Synced
    Is it a good idea to have devices
    which are so easy to hide
  • Not Synced
    and so easy to use?
  • Not Synced
    We recently heard that the FDA plans
    to intoduce stricter regulations
  • Not Synced
    on sales of these devices
    that contain e-liquid flavors
  • Not Synced
    in retail locations like convenience
    stores and gas stations,
  • Not Synced
    and also introduce stricter regulations
    on sales of devices to minors
  • Not Synced
    over the internet.
  • Not Synced
    Is this going to be sufficient to change
    this rapid increase in youth uptake?
  • Not Synced
    We need to ask and answer
    such critical questions.
  • Not Synced
    Now is also the time for a serious
    public education campaign.
  • Not Synced
    Teens and their parents need to know
  • Not Synced
    that while e-cigarettes may contain
    less toxins than cigarettes,
  • Not Synced
    they're certainly not benign.
  • Not Synced
    Exposure of their bodies to the chemicals
    produced by these devices
  • Not Synced
    could be changing them in ways
    they may not like
  • Not Synced
    and setting them up for future
    unknown toxicities and health problems.
  • Not Synced
    You know, when I said earlier
  • Not Synced
    that e-cigarettes were the perfect
    fit for the smarthphone generation,
  • Not Synced
    I was not kidding.
  • Not Synced
    We live in a technology-crazed world,
  • Not Synced
    where the latest device and technology
    gets a lot of attention.
  • Not Synced
    Just because it is technology
    and because it is the latest thing.
  • Not Synced
    More and more over the next few years
    and for the rest of our lives,
  • Not Synced
    we are going to see technologies
    coming into the marketplace
  • Not Synced
    that may not raise
    any health flags are first
  • Not Synced
    simply because they don't look unhealthy
  • Not Synced
    or they're not a medical device.
  • Not Synced
    For example, we could see devices
  • Not Synced
    that may make it easier
    to go longer without sleep
  • Not Synced
    or help us lose weight --
  • Not Synced
    a personal goal of mine --
  • Not Synced
    or achieve any number of other goals
  • Not Synced
    that we as consumers
    are very, very interested in.
  • Not Synced
    But many of these devices may come
    with unacceptable risks to our own health.
  • Not Synced
    So if we want to protect our health
    and the health of our children,
  • Not Synced
    perhaps we should get out of the habit
  • Not Synced
    of automatically celebrating
    such new technology
  • Not Synced
    and get into the habit of looking
    at them with a critical eye,
  • Not Synced
    perhaps even through a medical lens.
  • Not Synced
    Because you know something?
  • Not Synced
    Our health,
  • Not Synced
    the health of our children
  • Not Synced
    and our future generations
  • Not Synced
    is far too valuable
    to let it go up in smoke
  • Not Synced
    or even in aerosol.
