Physical therapy is boring — play a game instead

0:01 - 0:06When I was growing up, I really
liked playing hide-and-seek a lot. -
0:06 - 0:10One time, though, I thought climbing
a tree would lead to a great hiding spot, -
0:10 - 0:13but I fell and broke my arm.
-
0:13 - 0:17I actually started first grade
with a big cast all over my torso. -
0:18 - 0:22It was taken off six weeks later,
but even then, I couldn't extend my elbow, -
0:22 - 0:25and I had to do physical therapy
to flex and extend it, -
0:25 - 0:28100 times per day, seven days per week.

0:29 - 0:32I barely did it, because
I found it boring and painful, -
0:32 - 0:36and as a result, it took me
another six weeks to get better. -
0:37 - 0:40Many years later, my mom
developed frozen shoulder, -
0:40 - 0:44which leads to pain
and stiffness in the shoulder. -
0:44 - 0:47The person I believed for half of my life
to have superpowers -
0:47 - 0:50suddenly needed help
to get dressed or to cut food. -
0:51 - 0:54She went each week to physical therapy,
but just like me, -
0:54 - 0:56she barely followed the home treatment,
-
0:56 - 0:59and it took her
over five months to feel better. -
1:00 - 1:03Both my mom and I
required physical therapy, -
1:03 - 1:05a process of doing a suite
of repetitive exercises -
1:05 - 1:10in order to regain the range of movement
lost due to an accident or injury. -
1:10 - 1:12At first, a physical therapist
works with patients, -
1:12 - 1:15but then it's up to the patients
to do their exercises at home. -
1:15 - 1:19But patients find physical therapy
boring, frustrating, confusing -
1:19 - 1:22and lengthy before seeing results.

1:22 - 1:27Sadly, patient noncompliance
can be as high as 70 percent. -
1:27 - 1:30This means the majority of patients
don't do their exercises -
1:30 - 1:34and therefore take
a lot longer to get better. -
1:34 - 1:37All physical therapists agree
that special exercises -
1:37 - 1:39reduce the time needed for recovery,
-
1:39 - 1:41but patients lack
the motivation to do them. -
1:42 - 1:47So together with three friends,
all of us software geeks, -
1:47 - 1:49we asked ourselves,
-
1:49 - 1:54wouldn't it be interesting if patients
could play their way to recovery? -
1:54 - 1:56We started building MIRA,
A P.C. software platform -
1:56 - 1:59that uses this Kinect device,
a motion capture camera, -
1:59 - 2:03to transform traditional exercises
into video games. -
2:04 - 2:08My physical therapist has already set up
a schedule for my particular therapy. -
2:08 - 2:11Let's see how this looks.

2:14 - 2:17The first game asks me
to fly a bee up and down -
2:17 - 2:19to gather pollen to deposit in beehives,
-
2:19 - 2:22all while avoiding the other bugs.
-
2:22 - 2:25I control the bee by doing
elbow extension and flexion, -
2:25 - 2:29just like when I was seven years old
after the cast was taken off. -
2:30 - 2:33When designing a game,
we speak to physical therapists at first -
2:33 - 2:36to understand what movement
patients need to do. -
2:36 - 2:38We then make that a video game
-
2:38 - 2:41to give patients simple,
motivating objectives to follow. -
2:41 - 2:43But the software is very customizable,
-
2:43 - 2:47and physical therapists can also
create their own exercises. -
2:47 - 2:49Using the software, my physical therapist
-
2:49 - 2:51recorded herself performing
a shoulder abduction, -
2:51 - 2:54which is one of the movements
my mom had to do -
2:54 - 2:55when she had frozen shoulder.
-
2:55 - 2:59I can follow my therapist's example
on the left side of the screen, -
2:59 - 3:03while on the right, I see myself
doing the recommended movement. -
3:03 - 3:05I feel more engaged and confident,
-
3:05 - 3:07as I'm exercising alongside my therapist
-
3:07 - 3:11with the exercises my therapist
thinks are best for me. -
3:11 - 3:14This basically extends the application
for physical therapists -
3:14 - 3:18to create whatever exercises
they think are best. -
3:18 - 3:22This is an auction house game
for preventing falls, -
3:22 - 3:25designed to strengthen muscles
and improve balance. -
3:25 - 3:28As a patient, I need to do
sit and stand movements, -
3:28 - 3:29and when I stand up,
-
3:29 - 3:32I bid for the items I want to buy.
-
3:32 - 3:34(Laughter)
-
3:34 - 3:37In two days, my grandmother
will be 82 years old, -
3:37 - 3:39and there's a 50 percent chance
for people over 80 -
3:39 - 3:41to fall at least once per year,
-
3:41 - 3:44which could lead to a broken hip
or even worse. -
3:44 - 3:48Poor muscle tone and impaired balance
are the number one cause of falls, -
3:48 - 3:52so reversing these problems
through targeted exercise -
3:52 - 3:54will help keep older people
like my grandmother -
3:54 - 3:57safer and independent for longer.

3:57 - 4:00When my schedule ends,
MIRA briefly shows me -
4:00 - 4:03how I progressed throughout my session.
-
4:04 - 4:06I have just shown you
three different games -
4:06 - 4:08for kids, adults and seniors.
-
4:08 - 4:11These can be used with orthopedic
or neurologic patients, -
4:11 - 4:14but we'll soon have options
for children with autism, -
4:14 - 4:16mental health or speech therapy.
-
4:17 - 4:19My physical therapist
can go back to my profile -
4:19 - 4:22and see the data gathered
during my sessions. -
4:22 - 4:25She can see how much I moved,
how many points I scored, -
4:25 - 4:27with what speed I moved my joints,
-
4:27 - 4:28and so on.
-
4:28 - 4:31My physical therapist can use all of this
to adapt my treatment. -
4:32 - 4:34I'm so pleased this version is now in use
-
4:34 - 4:37in over 10 clinics
across Europe and the U.S., -
4:37 - 4:39and we're working on the home version.
-
4:39 - 4:44We want to enable physical therapists
to prescribe this digital treatment -
4:44 - 4:47and help patients play their way
to recovery at home. -
4:48 - 4:51If my mom or I had a tool like this
when we needed physical therapy, -
4:51 - 4:55then we would have been more successful
following the treatment, -
4:55 - 4:58and perhaps gotten better a lot sooner.

and how much does it cost? -

a Microsoft Surface Pro 3 for the demo, -
5:13 - 5:17but you just need a computer
and a Kinect, which is 120 dollars. -
5:17 - 5:20TR: Right, and the Kinect is the thing
that people use for their Xboxes -
5:20 - 5:21to do 3D games, right?

5:21 - 5:24CM: Exactly, but you don't need the Xbox,
you only need a camera. -
5:24 - 5:27TR: Right, so this is less
than a $1,000 solution. -
5:27 - 5:29CM: Definitely, 400 dollars,
you can definitely use it. -
5:29 - 5:32TR: So right now, you're doing
clinical trials in clinics. -
5:32 - 5:33CM: Yes.

5:33 - 5:36TR: And then the hope is to get it
so it's a home version -
5:36 - 5:37and I can do my exercise remotely,

5:37 - 5:41and the therapist at the clinic
can see how I'm doing and stuff like that. -
5:41 - 5:42CM: Exactly.
