Caesarean Section: Step 4 - Delivery of the baby, placenta

0:00 - 0:03The next step is to deliver the fetus

0:03 - 0:08where we would insert our hand below the head
-
0:08 - 0:10and we remove the retractor
-
0:11 - 0:14and as soon as the head is delivered
-
0:14 - 0:17before the delivery of the shoulder
-
0:17 - 0:21we would be first suctioning the mouth
-
0:21 - 0:23followed by the nostrils
-
0:24 - 0:24suctioning the mouth first would decrease the risk of aspiration
-
0:24 - 0:24and then we would deliver the whole baby
-
0:24 - 0:27and continue with the suction
-
0:27 - 0:31while we are clamping the cord
-
0:31 - 0:33the chord would be clamped

0:33 - 0:36at two points
-
0:36 - 0:45and would be cut by the assistant and the baby would be

0:45 - 0:47taken to continuing resusitation

0:47 - 0:50the next step now is to deliver the placenta
-
0:50 - 0:56theh placenta can be delivered by manual removal or controlled contraction
-
0:56 - 1:02the risk of bleeding and the risk of infection is found to be higher in removing it by manually
-
1:02 - 1:05so the preferable way is to remove it by controlled contraction, as you can see here

1:05 - 1:14also when the baby is being deivered
-
1:14 - 1:18we should give oxytocin to the mother
-
1:18 - 1:27and also during the delivery of the fetus also to give prophylactic antibiotics

1:27 - 1:33once we deliver the placenta
-
1:33 - 1:39it is always good to mop the inside of the uterine cavity
-
1:39 - 1:45to be sure we have not left any fragments of placenta or membranes
-
1:45 - 1:57and then we'll exteriorize the uterus to, both for easy visualization of the repair and also to see any bleeding,
-
1:57 - 2:04we should insert the X retractor at this point and we should look for any bleeders

2:04 - 2:12and this is where we are going to use a green armitage that you can see here holding, the surgeon holding

2:12 - 2:17and the green armitage is specific used for cesarean section because it is a clamp that can bring hemostasis without crushing the uterine wall
-
2:17 - 2:27all the bleeders would be clamped by the green armitage before we start
-
2:27 - 2:30the closure, closing the uterine incision
-
2:30 - 2:33This is the stage where we are going to dilate the cervix
-
2:33 - 2:40so we have good drainage
-
2:40 - 2:44using either fingers or sponge x
