have triple the lifetime risk
of heart disease and lung cancer

and a 20-year difference
in life expectancy.

And yet, doctors today are not trained
in routine screening or treatment.

Now, the exposure I'm talking about
is not a pesticideor packaging chemical.

Not Synced
It's childhood trauma.

Not Synced
Okay. What kind of trauma
am I talking about here?

Not Synced
I'm not talking about failing a test
-
Not Synced
or losing a basketball game.
-
Not Synced
I am talking about threats
that are so severe or pervasive
-
Not Synced
that they literally get under our skin
and change our physiology:

Not Synced
things like abuse or neglect,
-
Not Synced
or growing up with a parent who struggles
-
Not Synced
with mental illness
or substance dependence.

Not Synced
Now, for a long time,
I viewed these things
-
Not Synced
in the way I was trained to view them,
-
Not Synced
either as a social problems
-- refer to social services --

Not Synced
or as a mental health problem
-
Not Synced
-- refer to mental health services.

Not Synced
And then something happened
to make me rethink my entire approach.

When I finished my residency,
I wanted to go someplace
-
Not Synced
where I felt really needed,
-
Not Synced
someplace where I could make a difference.

Not Synced
So I came to work
for California Pacific Medical Center,
-
Not Synced
one of the best private hospitals
in Northern California,
-
Not Synced
and together, we opened a clinic
in Bay View Hunter's Point,
-
Not Synced
one of the poorest, most under-served
neighborhoods in San Francisco.
-
Not Synced
Now, prior to that point,
-
Not Synced
there had been only
one pediatrician in all of Bay View
-
Not Synced
to serve more than 10,000 children,
-
Not Synced
so we hung a shingle, and we were able
to provide top quality care
-
Not Synced
regardless of ability to pay.
-
Not Synced
It was so cool. We targeted
the typical health disparities:
-
Not Synced
access to care, immunization rates,
asthma hospitalization rates,
-
Not Synced
and we hit all of our numbers.
-
Not Synced
We felt very proud of ourselves.
-
Not Synced
But then I started noticing
a disturbing trend.
-
Not Synced
A lot of kids were being
referred to me for ADHD,
-
Not Synced
or Attention Deficit
Hyperactivity Disorder.
-
Not Synced
But when I actually did
a thorough history and physical,
-
Not Synced
what I found was that
for most of my patients,
-
Not Synced
I couldn't make a diagnosis of ADHD.
-
Not Synced
Most of the kids I was seeing
had experienced such severe trauma
-
Not Synced
that it felt like something else
was going on.
-
Not Synced
Somehow I was missing something important.
-
Not Synced
Now, before I did my residency,
I did a masters degree in public health,
-
Not Synced
and one of the things that they teach you
in public health school
-
Not Synced
is that if you're a doctor
-
Not Synced
and you see a hundred kids
that all drink from the same well,
-
Not Synced
and 98 of them develop diarrhea,
-
Not Synced
you can go ahead
and write that prescription
-
Not Synced
for dose after dose
after dose of antibiotics,
-
Not Synced
or you can walk over and say,
"What the hell is in this well?"
-
Not Synced
So I began reading everything
I could get my hands on
-
Not Synced
about how exposure to adversity
-
Not Synced
affects the developing brains
and bodies of children.

Not Synced
And then one day,
-
Not Synced
my colleague walked into my office,
-
Not Synced
and he said, "Dr. Burke,
have you seen this?"
-
Not Synced
In his hand was a copy
of a research study
-
Not Synced
called the Adverse Childhood
Experiences Study.
-
Not Synced
That day changed my clinical practice
-
Not Synced
and ultimately my career.
-
Not Synced
The Adverse Childhood Experiences Study
-
Not Synced
is something that everybody
needs to know about.
-
Not Synced
It was done by Dr. Vince Felitti at Kaiser
and Dr. Bob Anda at the CDC,
-
Not Synced
and together, they asked
17 and a half thousand adults
-
Not Synced
about their history of exposure
-
Not Synced
to what they called
"adverse childhood experiences,"
-
Not Synced
or ACEs.
-
Not Synced
Those include physical, emotional,
or sexual abuse;
-
Not Synced
physical or emotional neglect;
parental mental illness;
-
Not Synced
substance dependence; incarceration;
-
Not Synced
parental separation or divorce;
-
Not Synced
or domestic violence.
-
Not Synced
For every yes, you would get
a point on your ACE score.
-
Not Synced
And then what they did
was they correlated these ACE scores
-
Not Synced
against health outcomes.
-
Not Synced
What they found was striking.
-
Not Synced
Two things: number one,
-
Not Synced
ACEs are incredibly common.
-
Not Synced
Sixty-seven percent of the population
-
Not Synced
had at least one ACE,
-
Not Synced
and 12.6 percent, one in eight,
-
Not Synced
had four or more ACEs.
-
Not Synced
The second thing that they found
-
Not Synced
was that there was
a dose response relationship
-
Not Synced
between ACEs and health outcomes:
-
Not Synced
the higher your ACE score,
the worse your health outcomes.
-
Not Synced
For a person with an ACE score
of four or more,
-
Not Synced
their relative risk of chronic
obstructive pulmonary disease
-
Not Synced
was two and a half times that
of someone with an ACE score of zero.
-
Not Synced
For hepatitis, it was also
two and a half times.
-
Not Synced
For depression, it was
four and a half times.
-
Not Synced
For suicidality, it was 12 times.

Not Synced
A person with an ACE score
of seven or more
-
Not Synced
had triple the lifetime risk
of lung cancer

and three and a half times the risk
of ischemic heart disease,

the number one killer
in the United States of America.

Well, of course this makes sense.
-
Not Synced
You know, some people looked at this data
and they said, "Come on.
-
Not Synced
You know, you have a rough childhood,
you're more likely to drink and smoke
-
Not Synced
and do all these things
that are going to ruin your health.
-
Not Synced
This isn't science.
This is just bad behavior."

Not Synced
It turns out this is exactly
where the science comes in.

Not Synced
We now understand
better than we ever have before
-
Not Synced
how exposure to early adversity
-
Not Synced
affects the developing brains
and bodies of children.
-
Not Synced
It affects areas like
the nuclea succumbun,
-
Not Synced
the pleasure and reward
center of the brain
-
Not Synced
that is implicated
in substance dependence.
-
Not Synced
It inhibits the prefrontal cortex,
which is necessary for impulse control
-
Not Synced
and executive function,
-
Not Synced
a critical area for learning.
-
Not Synced
And on MRI scans,
-
Not Synced
we see measurable differences
in the amygdala,
-
Not Synced
the brain's fear response center.
-
Not Synced
So there are real neurologic reasons
-
Not Synced
why folks exposed
to high doses of adversity
-
Not Synced
are more likely to engage
in high-risk behavior,
-
Not Synced
and that's important to know.
-
Not Synced
But it turns out that even if you don't
engage in any high risk behavior,
-
Not Synced
you're still more likely
-
Not Synced
to develop heart disease or cancer.
-
Not Synced
The reason for this has to do with
-
Not Synced
the hypothalamic–pituitary–adrenal axis,
-
Not Synced
the brain's and body's
stress response system
-
Not Synced
that governs our fight-or-flight response.
-
Not Synced
How does it work?
-
Not Synced
Well, imagine you're walking
in the forest and you see a bear.
-
Not Synced
Immediately, your hypothalamus
sends a signal to your pituitary,
-
Not Synced
which sends a signal to your adrenal gland
-
Not Synced
that says, "Release stress hormones!
Adrenaline! Cortisol!"
-
Not Synced
And so your heart starts to pound,

Not Synced
Your pupils dilate, your airways open up,
-
Not Synced
and you are ready
-
Not Synced
to either fight that bear
or run from the bear.

Not Synced
And that is wonderful
-
Not Synced
if you're in a forest
-
Not Synced
and there's a bear.
-
Not Synced
(Laughter)

But the problem is what happens

when the bear comes home every night,
-
Not Synced
and this system is activated
-
Not Synced
over and over and over again,
-
Not Synced
and it goes from being adaptive
-
Not Synced
or life-saving
-
Not Synced
to maladaptive or health-damaging.
-
Not Synced
Children are especially sensitive
to this repeated stress activation,
-
Not Synced
because their brains and bodies
are just developing.
-
Not Synced
High doses of adversity not only affect
brain structure and function,
-
Not Synced
they affect the developing immune system,
-
Not Synced
developing hormonal systems,
-
Not Synced
and the even the way our DNA
is read and transcribed.

So for me, this information
threw my old training out the window,

Not Synced
because when we understand
the mechanism of a disease,
-
Not Synced
when we know not only
which pathways are disrupted, but how,
-
Not Synced
then as doctors, it is our job
to use this science
-
Not Synced
for prevention and treatment.

Not Synced
That's what we do.

So in San Francisco, we created
the Center for Youth Wellness
-
Not Synced
to prevent, screen, and heal the impacts
of ACEs and toxic stress.
-
Not Synced
We started simply with routine screening
of every one of our kids
-
Not Synced
at their regular physical,
-
Not Synced
because I know
-
Not Synced
that if my patient has an ACE score of 4,
-
Not Synced
she's two and a half times as likely
to develop hepatitis or COPD,
-
Not Synced
she's four and half times as likely
to become depressed,
-
Not Synced
and she's 12 times as likely
to attempt to take her own life
-
Not Synced
as my patient with zero ACEs.
-
Not Synced
I know that when she's in my exam room.
-
Not Synced
For our patients who do screen positive,
-
Not Synced
we have a multidisciplinary treatment team
-
Not Synced
that works to reduce the dose of adversity
-
Not Synced
and treat symptoms using best practices,
-
Not Synced
including home visits, care coordination,
-
Not Synced
mental health care, nutrition,
-
Not Synced
holistic interventions, and yes,
medication when necessary.

Not Synced
But we also educate parents
about the impacts of ACEs and toxic stress
-
Not Synced
the same way you would
for covering electrical outlets
-
Not Synced
or lead poisoning,
-
Not Synced
and we tailor the care
of our asthmatics and our diabetics
-
Not Synced
in a way that recognizes that they may
need more aggressive treatment,
-
Not Synced
given the changes to their hormonal
and immune systems.

Not Synced
So the other thing that happens
when you understand this science
-
Not Synced
is that you want to shout it
from the rooftops,
-
Not Synced
because this isn't just an issue
for kids in Bay View.
-
Not Synced
I figured the minute
that everybody else heard about this,
-
Not Synced
it would be routine screening,
multi-disciplinary treatment teams,
-
Not Synced
and it would be a race to the most
effective clinical treatment protocols.

Not Synced
Yeah. That did not happen.
-
Not Synced
And that was a huge learning for me.
-
Not Synced
What I had thought of as simply
best clinical practice
-
Not Synced
I now understand to be a movement.

Not Synced
In the words of Dr. Robert Block,

the former President
of the American Academy of Pediatrics,

"Adverse childhood experiences

are the single greatest
unaddressed public health treat

facing our nation today."

And for a lot of people,
that's a terrifying prospect.

The scope and scale of the problem
seems so large that it feels overwhelming
-
Not Synced
to think about how we might approach it.

Not Synced
But for me, that's actually
where the hopes lies,

Not Synced
because when we have the right framework,
-
Not Synced
when we recognize this to be
a public health crisis,
-
Not Synced
then we can begin
to use the right tool kit
-
Not Synced
to come up with solutions.
-
Not Synced
From tobacco to lead poisoning to HIV/AIDS,
-
Not Synced
the United States actually has
-
Not Synced
quite a strong track record
-
Not Synced
with addressing public health problems,
-
Not Synced
but replicating those successes
with ACEs and toxic stress
-
Not Synced
is going to take determination
and commitment,
-
Not Synced
and when I look at what
our nation's response has been so far,
-
Not Synced
I wonder,
-
Not Synced
why haven't we taken this more seriously?
-
Not Synced
You know, at first I thought
that we marginalized the issue
-
Not Synced
because it doesn't apply to us.
-
Not Synced
Right? That's an issue for those kids
in those neighborhoods.
-
Not Synced
Which is weird, because the data
doesn't bear that out.
-
Not Synced
The original ACEs study
was done in a population
-
Not Synced
that was 70 percent Caucasian,
-
Not Synced
70 percent college-educated.
-
Not Synced
But then, the more I talked to folks,
-
Not Synced
I'm beginning to think that maybe
I had it completely backwards.
-
Not Synced
If I were to ask how many people
in this room grew up
-
Not Synced
with a family member
who suffered from mental illness,
-
Not Synced
I'll bet a few hands would go up,
-
Not Synced
and then if I were to ask how many folks
had a parent who maybe drank too much,
-
Not Synced
or who really believed that
if you spare the rod, you spoil the child,
-
Not Synced
I bet a few more hands would go up.

Even in this room, this is an issue
that touches many of us,
-
Not Synced
and I am beginning to believe
that we marginalize the issue
-
Not Synced
because it does apply to us.

Not Synced
Maybe it's easier to see
in other zip codes
-
Not Synced
because we don't want to look at it.
-
Not Synced
We'd rather be sick.
-
Not Synced
Fortunately, scientific advances
-
Not Synced
and, frankly, economic realities
-
Not Synced
make that option less viable every day.
-
Not Synced
The science is clear:
-
Not Synced
early adversity dramatically affects
health across a lifetime.
-
Not Synced
Today, we are beginning to understand
how to interrupt the progression
-
Not Synced
from early adversity
to disease and early death,
-
Not Synced
and 30 years from now,
-
Not Synced
the child who has a high ACE score
-
Not Synced
and whose behavioral symptoms
go unrecognized,
-
Not Synced
whose asthma management
is not connected,
-
Not Synced
and who goes on to develop
high blood pressure
-
Not Synced
and early heart disease or cancer
-
Not Synced
will be just as anomalous
as a six month mortality from HIV/AIDS.
-
Not Synced
People will look at that situation
and say, "What the heck happened there."
-
Not Synced
This is treatable.

Not Synced
This is beatable.

Not Synced
The single most important thing
that we need today

is the courage to look
this problem in the face

and say this is real

and this is all of us.
