Myomectomy - Repairing the myoma bed

0:05 - 0:19In this video as it is quite visible, once the myoma is removed or taken out

0:19 - 0:22then a big cavity will be formed where the myoma has been

0:22 - 0:27existing. The initial step in the repair of the myoma is the

0:27 - 0:31repair the bed of the of the myoma

0:31 - 0:35which is the bed where the myoma has been existing.

0:35 - 0:43Unless this cavity is effectively obliterated by strong stitch like Vicryl #2 or #1
-
0:43 - 0:50then it will collect bleeders from the surface
-
0:50 - 0:52from the bed then it will cause hemorrhage
-
0:52 - 0:57which is unwanted side effect of the surgery.

0:57 - 1:01So to avoide such hemorrhage into the cavity or the bed of the myoma,
-
1:01 - 1:12we have to ??? surrounding suture using Vicryl stitch is #1 or #2 of choice

1:12 - 1:21and once the myoma cavity is obliterated
-
1:21 - 1:27effectively, the rest of the uterus or the uterine incision should be repaired
-
1:27 - 1:33in layers. Which means the myometrium should be
-
1:33 - 1:38sutured by Vicryl stitch running, and the
-
1:38 - 1:45serosa should also be sutured by Vicryl stitch #0, using the
-
1:45 - 1:50baseball technique of approximating the serosa
-
1:50 - 1:54of the uterus.

1:54 - 1:59Effective covering up of the uterine incision by the serosa layer is important because
-
1:59 - 2:09during the process of healing it may form adhesion with the bowel, with the bladder
-
2:09 - 2:14and subsequently if the woman is to have a
-
2:14 - 2:18caesarean section or repeat myomectomy it is going to be difficult
-
2:18 - 2:25so the initial surgery should be properly closed
-
2:25 - 2:31Once the uterine incisions are done
-
2:31 - 2:37the surgeon has to make sure that hemostasis is effectively controlled
-
2:37 - 2:41which means there is no bleeders from the incision site.
-
2:41 - 2:46And if there are any, then it should be strengthened
-
2:46 - 2:51by applying uninterrupted figure 8 stitch at the site of the bleeder
-
2:51 - 2:57or by applying cautery. Once the bleeding
-
2:57 - 3:05is controlled, the uterus can be replaced back
-
3:05 -into the abdominal cavity for abdominal wall repair.
