0:19 - 0:20she's a registered dietician

0:20 - 0:23and i know i told you guys i was gonna sit down with her
-
0:23 - 0:24and i know it seems like it's been forever
-
0:24 - 0:27but we finally got our schedules to work.
-
0:27 - 0:34today's topic is just kind of demystifying what it means to go see a dietitian
-
0:34 - 0:38i know many of you are scared to call a therapist and are scared to see our doctor
-
0:38 - 0:42even makign an appointment can send us into a frenzy
-
0:42 - 0:46so we want to picture that we feel okay
-
0:46 - 0:50and we know what to expect because a lot of time we're getting anxious
-
0:50 - 0:53because of the unknown, it's scary when we don't know
-
0:53 - 0:57so i guess my question for kate today is
-
0:57 - 1:02just tell us a little bit about what the first session will look like
-
1:02 - 1:05obviously there's a lot of different ways
-
1:05 - 1:11but i think the first thing to think about it how to find a deitian that specialices
-
1:11 - 1:17in eating disorder in your area or in the area that your eating disorder is in
-
1:17 - 1:19because the ore specific the better
-
1:19 - 1:23but it just really depends on the individual
-
1:23 - 1:27so like you were saying it is really...
-
1:27 - 1:30anything new can be anxiety provoking
-
1:30 - 1:32so looking for a dietian...well how do you judge?
-
1:32 - 1:36you base them on their name, on their location
-
1:36 - 1:38on the picture you see on the internet
-
1:38 - 1:40none of that is going to be a good indicator
-
1:40 - 1:43the best way to find a dietician is through a referral
-
1:43 - 1:47whether you're seeing a therapist who can reger you to one that they work closely with
-
1:47 - 1:49or a doctor can refer
-
1:49 - 1:51there are a lot of really good websites out there
-
1:51 - 1:54kati probably has this on her references page
-
1:54 - 1:58that you can look for to refer to specialists in eating disorders
-
1:58 - 2:04and then make a phone call to see if you can have a good phone call
-
2:04 - 2:09if you don't feel comfortable, you're interviewing them first
-
2:09 - 2:12if you don't like the way it's going say
-
2:12 - 2:16"well thank you, i don't want to make a time"
-
2:16 - 2:20if you do want to go forward and make a first appointment
-
2:20 - 2:23just set that up and the first appointment
-
2:23 - 2:28and right now i'm kind of talking about outpatient basis first
-
2:28 - 2:30and then i'll get to an inpatient basis
-
2:30 - 2:32so if you want to make an appointment outpatient
-
2:32 - 2:38the first appointment is usually between 1 and 1 and a half hours
-
2:38 - 2:41and that will generally cover everything
-
2:41 - 2:44from the history of your eating disorder,
-
2:44 - 2:45how long you've had it
-
2:45 - 2:49what kind of treatment you might of had in the past
-
2:49 - 2:52what behvaviors you are currently dealing with
-
2:52 - 2:53what you've did in the past
-
2:53 - 2:59it will definitely include a lot about food and possibly your weight
-
2:59 - 3:03and the history of maybe your physical changes
-
3:03 - 3:04or any kind of medical conditions you've had
-
3:04 - 3:09so that's usually as i found the most fear inducing part
-
3:09 - 3:11for a lot of new clients that come in
-
3:11 - 3:15because it's really personal to tell this stuff to someone you don't know
-
3:15 - 3:19especially since often eating disorders are kind of kept private
-
3:19 - 3:23kept secret, so then telling someone that you're not that comfortable with
-
3:23 - 3:25all of your secrets
-
3:25 - 3:30and then i know there's a really big fear of being honest
-
3:30 - 3:34because what if we are judgemental, what if we come across as
-
3:34 - 3:37"oh my gosh, that is so disgusting"
-
3:37 - 3:38"i can't believe you've done that"
-
3:38 - 3:42and any good dietian i know that works with eating disorders
-
3:42 - 3:47has no judgement when dealing with clients or treating clients
-
3:47 - 3:52there is probably nothing we haven't heard before
-
3:52 - 3:54not just that we haven't heard before
-
3:54 - 3:57but it's part of the eating disorder
-
3:57 - 4:01and everbody's different and is going to experience behaviors differently
-
4:01 - 4:07and most imporatn thing is you can be honest to us
-
4:07 - 4:12because as a dietiaitan who may be addressing specidic issues
-
4:12 - 4:16when it comes to how much were you really eating before
-
4:16 - 4:17and what foods can you eat
-
4:17 - 4:21and what foods make you sick or have really intense reactions to
-
4:21 - 4:23in terms of psychological or physical
-
4:23 - 4:27but if you do not tell the whole truth
-
4:27 - 4:32i can go around in circles and give you all this good advice i think i have
-
4:32 - 4:38that really is useless because it may not be based on what truth you've told me
-
4:38 - 4:44for me that's probably the biggest part of the first session
-
4:44 - 4:45having someone come in
-
4:45 - 4:48they may have their general data
-
4:48 - 4:53age, height, kind of basic history of themselves
-
4:53 - 5:00and then it would be applying the program to their situation at the time
-
5:00 - 5:04but i can't do that unless i'm feeling like that person is willing to try
-
5:04 - 5:08at least and open to some of the suggestions
-
5:08 - 5:10so that might mean someone's coming in
-
5:10 - 5:15maybe they've had an eating disorder for a year
-
5:15 - 5:17and it's been getting a lot worse
-
5:17 - 5:19and they're not eating much at all
-
5:19 - 5:23on a daily basis, or keeping a lot in on a daily basis
-
5:23 - 5:26so it might just be increasing the amount of times you eat
-
5:26 - 5:28a little bit at a time
-
5:28 - 5:30and that can be really scary
-
5:30 - 5:33but any first session would just be about
-
5:33 - 5:35really small steps in the beginning
-
5:35 - 5:38in terms of an outpatient basis
-
5:38 - 5:40and hten we would see how that goes for the first wek
-
5:40 - 5:45and at that point i would recommend a weekly reoccuring appointment
-
5:45 - 5:48until there's progress beign made
-
5:48 - 5:52and hten kind of on the flipside
-
5:52 - 5:57if i'm doing an intial session in more a treatment center session
-
5:57 - 6:02a lot of the backgroung info i may have gotten from the other treatment team members
-
6:02 - 6:06and yes i would collect that info again
-
6:06 - 6:07just to get to know the client and
-
6:07 - 6:12hopefully the client will like to be working in this kind of manner
-
6:12 - 6:17and thenreally star addressing the food iissuues
-
6:17 - 6:21when it comes to how much, when, where to eat, what types of food to eat
-
6:21 - 6:25how to do it, and really practical information
-
6:25 - 6:29i think that can be one of the hardest things
-
6:29 - 6:32when it comes to therapy or treatment
-
6:32 - 6:34a lot of it seems big scope like
-
6:34 - 6:37"well just get over your anxiety" and
-
6:37 - 6:40"just eat again it's really not a probelm"
-
6:40 - 6:41but it really comes down to specifics like
-
6:41 - 6:44"you have to eat this thing at this amount at this time
-
6:44 - 6:47and then tell me about it, what did you feel"
-
6:47 - 6:53so it's not like youre doing it completely on your own with no support
-
6:53 - 6:55i think our biggest job is
-
6:55 - 6:59we might put out some recommendations and then seeing how you do with it right away
-
6:59 - 7:05and then addressing everything from there based on those first experiences
-
7:05 - 7:12but i think for me when i see new patients in whatever kind of level
-
7:12 - 7:19i would say a good part of any first session is building a repoir with the client
-
7:19 - 7:23and getting to know them because everyone is different
-
7:23 - 7:28when it comes to how they take advice and how comfortable they are with speaking to someone else
-
7:28 - 7:33and so it may seem really easy the first session
-
7:33 - 7:36cause it's not that hard
-
7:36 - 7:38unless you're not comfortable talking to somebody
-
7:38 - 7:42but that's kind of the benefit of doing this video right now
-
7:42 - 7:47to understand what it will feel like and preparing for that first session
-
7:47 - 7:52yeah definitely and like kate said everybody is different
-
7:52 - 7:56so any first session we're triyg to get to know you
-
7:56 - 7:59make sure you're comfrratble, make sure we have a good relationship
-
7:59 - 8:02because we're working togeteher
-
8:02 - 8:05i'm always saying "we" and "us" because it's a team
-
8:05 - 8:06we're workin with you
-
8:06 - 8:08you're not alone and thngs will be hard
-
8:08 - 8:09and we're going to challenge you
-
8:09 - 8:11because otherwise we're not doing our job
-
8:11 - 8:13and we're not a good part of your team
-
8:13 - 8:17so it might feel like we're pushing you a little bit
-
8:17 - 8:21but it's just to see the honest input from you
-
8:21 - 8:25like "well i tried this and this is how i felt and i couldn't do it"
-
8:25 - 8:32because then we take that and we pput it back in our "machine"
-
8:32 - 8:35and we try to come up with another option
-
8:35 - 8:42that's the importance in seeing an individual dietitian
-
8:42 - 8:46or in person because they can take what you're sayin in the moment
-
8:46 - 8:48or what's going on in your life in that present time
-
8:48 - 8:51and gind a way for you to get through it
-
8:51 - 8:55and for me it's with food, or it's with eating and getting the support you need to eat
-
8:55 - 9:01but it's really doable and as many times as i
-
9:01 - 9:04ive heard that patients arent looking forward to seeing me
-
9:04 - 9:11or that they're really upset that oyou've given them more food to eat
-
9:11 - 9:13i have no hard feelings with anybody
-
9:13 - 9:17becauase when sobmed goes through and coes out the other end of recoverey
-
9:17 - 9:21they are so greatful tat i was there and we just laught about
-
9:21 - 9:27how mean they were to me and how they were really afraid of me
-
9:27 - 9:29and so none of hta scares me, it doesnt scare me away
-
9:29 - 9:34doesn't oush me away, doesn't make me dislike somebody
-
9:34 - 9:37it's just hte aeating disrorder
-
9:37 - 9:41and that's what i'm daelintg with so i'm not intimidated by that at all
-
9:41 - 9:45and i think that any dietieian who's experience with eating disorders is the same exact way
-
9:45 - 9:49more than anything we just want to help
-
9:49 - 9:53but some of the biggest fears we deal with is
-
9:53 - 9:55"well how much am i going to have to eat,
-
9:55 - 9:56"am i going to get fat?
-
9:56 - 10:00"what does she know about me, she's blah blah blah, looks a certain way"
-
10:00 - 10:06and a lot of what we get is "what are you eating and why do you do that"
-
10:06 - 10:14and obviously that's just the eating disorder so we try to turn that back around and work with the client
-
10:14 - 10:17and really just address sombody where they're at
-
10:17 - 10:23yeah and those will be all the other questions we answer with kate
-
10:23 - 10:24i know that you have those concerns
-
10:24 - 10:28and we'l talk abotu them and adres them and kind of show how we use them
-
10:28 - 10:32in our sessions and what she does when she works with you developing meal plans
-
10:32 - 10:35and what that looks like on the other side
-
10:35 - 10:39so i hope that was informational and we'll keep doing more of these videos

This week's video has to do with demystifying the dietitian and what that first appointment with them would look like. Just like making an appointment with a therapist you have to give us a call and make sure that you are comfortable seeing us. If you like the way that they sound on the phone and you get a good feel for them, than you can set up a time. If not just let them know that you will contact them if you are ready to set up an appointment. Once you get to that appointment, the dietitian will be looking to gather some history about your relationship with food, treatment history, any health issues, behaviors with food, when they started, etc. Their focus is food and how we act around it, and although there will be times when it feels like they are just focusing on the small things, it is all of those small items that add up to recovery.
Every week when you see them they will check in with you and see how everything went with the food and what you were able to do and not do. That is why it is so important that we are honest with our dietitian. If we give her false information, than she could be going round and round with us thinking that certain things are not a problem, and therefore not really being helpful to us. So just do your best to be honest, tell her about your struggles and things that you simply couldn't do. Then we have more information that we can take back and come up with a new and better plan for you.
If you see your first dietitian in an inpatient or residential clinic than it will look very similar, but most of the history and health questions will have already been gathered by other treatment team members. Also, we tend to move a little bit faster while in residential or inpatient because there is 24 hour support available and we are having process groups every day. Having all of that extra support and supervision allows us to meet goals and work through the tough situations more quickly. I hope that this video helps demystify what it will look like to make an appointment with a dietitian and in turn help us keep moving towards a Healthy Mind and a Healthy Body. - Duration:
