for my parents. -

0:24 - 0:27you should have chosen
a medical specialty to save lives; -
0:27 - 0:31instead, you'll be looking
at warts and lumps your whole life". -
0:31 - 0:34I'd like to show you three things:

0:34 - 0:37First, my mother was mistaken,
-
0:38 - 0:39and second,
-
0:40 - 0:41dermatology is a great help
-
0:41 - 0:44changing people's lives
through the skin. -
0:44 - 0:46And third,
-
0:46 - 0:48a doctor who only knows about medicine
-
0:48 - 0:50doesn't know anything in fact.
-
0:50 - 0:52Why did I choose dermatology?
-
0:52 - 0:54When I was a medical student
-
0:54 - 0:57at the hospital, we went
to see a patient at the ICU. -
0:57 - 0:59The patient was in a bad state,
-
0:59 - 1:01with injuries all over the body;
the prognostic was bad. -
1:01 - 1:03and he had also ulcers on his legs.
-
1:03 - 1:06The reason why
a dermatologist was called. -
1:06 - 1:09The dermatologist explored
the patient and said, -
1:09 - 1:13"The patient is allergic to one
of the creams that you are using." -
1:13 - 1:17Two days later, the team at the hospital
stopped the cream and he got better. -
1:17 - 1:19I though that was incredible.

1:19 - 1:22A specialization, that within seconds,
allows you to make a diagnosis, -
1:22 - 1:25to start a treatment
and to help a patient. -
1:25 - 1:26Let see, for example, Ines.

1:26 - 1:28Ines came to our practice
-
1:28 - 1:31because she had a redness
over their eyelids, and was very tired. -
1:31 - 1:33She was diagnosed with dermatomyositis,
-
1:33 - 1:35a sort of illness associated
-
1:35 - 1:36with some cancers.
-
1:36 - 1:38It turned to be true;
Inés had breast cancer. -
1:38 - 1:42She was treated and saved, because
the cancer was in the early stages. -
1:42 - 1:44It was possible to help Ines,
thanks to her skin. -
1:44 - 1:46Another example, Beatriz,
came with these spots. -
1:46 - 1:48These white spots are known as vitiligo.
-
1:48 - 1:51Beatriz was diagnosed
with an illness of thyroid, -
1:51 - 1:54sometimes associated with vitiligo.
-
1:55 - 1:59Lucia: Lucia came to our practice because
she had these coffee milk spots -
1:59 - 2:01and they were caused
by a neurofibromatocis, -
2:01 - 2:04an illness associated with brain tumours.
-
2:04 - 2:08Lucía had a little cerebral tumor,
which was treated and cured. -
2:08 - 2:10Dermatologists, we can
-
2:10 - 2:14diagnose many illnesses through skin.
-
2:15 - 2:18But it is possible not only through
the skin, but also through hair and nails. -
2:18 - 2:23Dermatology is the external face
of the internal medicine. -
2:23 - 2:26In fact, dermatologists are
the managers of the imperfections, -
2:26 - 2:29some of them medical,
and some others aesthetics. -
2:29 - 2:32Skin cancer is one of worst imperfections.

2:33 - 2:34Did you know that here,

2:34 - 2:38four in ten people
will be affected by skin cancer? -
2:39 - 2:41The statistics are harsh.

2:41 - 2:43In the United States, every minute,

2:43 - 2:45a melanoma case is diagnosed.

2:45 - 2:48And every hour a patient dies
due to a melanoma -
2:48 - 2:50because it is one
of the most aggressive cancers. -
2:51 - 2:53Here is the case of Mateo
who came to our practice -
2:53 - 2:55because of one melanoma on his back.
-
2:55 - 2:58The melanoma was extirpated,
but the injury was very deep -
2:58 - 3:00and the tumour has already spread
-
3:00 - 3:02to the brain and other organs.
-
3:02 - 3:04Later, came his brother,
-
3:04 - 3:06who was very anxious about that,
-
3:06 - 3:07and his own back
-
3:07 - 3:09which was full of freckles.
-
3:09 - 3:11One of them was a melanoma.
-
3:11 - 3:12Which?
-
3:12 - 3:14It is difficult to know at first glance;

3:14 - 3:15this was the one.

3:15 - 3:18Then, with a machine

3:20 - 3:22we can see the freckles inside.

3:33 - 3:37around 100% of the cases
are cured if diagnosed on time. -
3:38 - 3:39Let's look at Julian.

3:39 - 3:42He came due to light acne.

3:42 - 3:45He had gone two years
without leaving home. -
3:45 - 3:48He had no friends, no studies,

3:48 - 3:50and he was deeply affected.

for hemangiomas 3 years ago. -

4:23 - 4:27It was possible to help him,
and to change life of the whole family, -
4:27 - 4:29through the improvements
with the modern dermatology -
4:30 - 4:32Maria came to our practice
-
4:32 - 4:34because she looked like
she was suffering from tonsillitis. -
4:34 - 4:41She started developing some illness
on the back, legs, arms and hair. -
4:42 - 4:43Maria had psoriasis,
-
4:44 - 4:47which affects 5% Spanish population.
-
4:48 - 4:51Maria started changing
her way of dressing, -
4:51 - 4:52going out less often,
-
4:52 - 4:54and avoiding the swimming
pool or the beach. -
4:54 - 4:58She even started to look for miraculous
treatments on the internet. -
4:58 - 5:00Finally she came to our practice
and received a treatment -
5:00 - 5:03known as "light therapy",
which is just taking sun. -
5:03 - 5:06Only with sun, the psoriasis disappeared.
-
5:06 - 5:08Now, Maria has a normal life
-
5:08 - 5:10and the psoriasis is under control.
-
5:10 - 5:14Before I carry on, I'd like to tell you
something that happened this morning. -
5:14 - 5:18I was running because I had not been
in Valladolid since I was a child, -
5:18 - 5:21so, I went running to river shore,
-
5:21 - 5:23and I found a beautiful landscape.
-
5:23 - 5:29You realize how, like the changing skin
of the banks of the river, -
5:29 - 5:32you are also changing people's lives here.
-
5:32 - 5:34People are practising paddle surf,
-
5:34 - 5:37canoeing, volley, practising sports.
-
5:37 - 5:39It's similar to Albania,
capital of Tirana, -
5:40 - 5:42which was a grey,
and sad city three years ago; -
5:42 - 5:45typical post-communism city,
-
5:45 - 5:47with lots of delinquency
-
5:47 - 5:52and the Mayor decided to paint
all houses in brilliant colours. -
5:53 - 5:56A month later,
-
5:58 - 6:00delinquency had fallen by a 90%.
-
6:01 - 6:04So, if we change the skin of things
-
6:04 - 6:06and that affects our behaviours,
-
6:06 - 6:09changing our skin will affect us greatly.
-
6:11 - 6:14I am going to tell you
the story of Adriana. -
6:15 - 6:19Adriana came to our practice
because as she said, -
6:19 - 6:21"Doctor, I do not recognise myself
-
6:21 - 6:25when I am watching myself in the mirror.
-
6:25 - 6:27What can I do?"
-
6:27 - 6:31The battle against aging
is a lost battle since we're born. -
6:33 - 6:35But it is also true
that modern dermatology -
6:35 - 6:37has some techniques
-
6:37 - 6:39that can help our patients
-
6:39 - 6:42to become old with elegance
and discretion. -
6:42 - 6:44We made a combination of treatments.

6:44 - 6:47She sent me an email
some months ago, telling me, -
6:47 - 6:49"My self-esteem is much better,
-
6:49 - 6:51my relationships have improved,
-
6:51 - 6:53and I have just been promoted at work."

6:53 - 6:54And the most important,

6:54 - 6:58"When watching myself in the mirror,
how I look matches how I feel." -
6:58 - 7:01It means that aesthetic dermatology,
as frivolous as it can sound, -
7:01 - 7:03can help people.

7:03 - 7:08It is true that if we're only looking
for an ideal of beauty, the final results -
7:08 - 7:10can be grotesque.
-
7:10 - 7:12And when looking around us,
-
7:12 - 7:14we can see very attractive people
-
7:14 - 7:16who have asymmetric eyebrows,
-
7:16 - 7:18big noses and prominent mouths;
-
7:18 - 7:21their faces are imperfect,
-
7:21 - 7:23however, they are very attractive.
-
7:23 - 7:25It is known that a smile
-
7:25 - 7:28is the best first impression,
-
7:28 - 7:30rather than what clothing
or surgery can do. -
7:30 - 7:33Even if the smile is squint.
-
7:33 - 7:36As happens with the smile
of my wife and my daughter Adriana. -
7:37 - 7:40Dermatologist has the duty
-
7:41 - 7:44to tell people when less is more,
-
7:44 - 7:47and that we have to live
with our physical imperfections. -
7:47 - 7:48Picasso said,

7:48 - 7:51"We have to look for beauty
within imperfection." -
7:51 - 7:54We should not be obsessed
with removing the wrinkles of life -
7:54 - 7:56but to put more life in our wrinkles.

7:57 - 8:01Did you know that many of the techniques
used in aesthetic dermatology -
8:01 - 8:04are also used in medical dermatology?

8:04 - 8:06An example is facial paralysis;

8:06 - 8:08There are many patients
with facial paralysis, -
8:08 - 8:10and thanks to Botox, which relax muscles,

8:10 - 8:12they can improve their condition,
-
8:12 - 8:13and he quality of their lives.

8:13 - 8:16Patients have more symmetrical faces.

8:17 - 8:18Or in the case of María José

8:18 - 8:20who came to our practice

8:20 - 8:22because she was suffering
from Hyperhidrosis. -
8:22 - 8:25Hyperdrosis is sweating
too much, by armpits or hands. -
8:25 - 8:27She destroyed the keyboard
on her computer, -
8:27 - 8:30her clothes, and she was very worried
about her wedding day. -
8:30 - 8:33Finally, she was injected with Botox

8:33 - 8:36and she could enjoy her happy wedding.

8:37 - 8:39What will the future of dermatology be?

8:39 - 8:41The future is fascinating.
-
8:42 - 8:44New and personalised drugs
are being developed, -
8:44 - 8:46according to patient's genetic code,
-
8:46 - 8:48or a tumor's genetic code.
-
8:48 - 8:50It is possible to make
a diagnosis via distance -
8:50 - 8:52thanks to teledermatology.
-
8:52 - 8:55We are collaborating
with some African organizations -
8:55 - 9:00and they send us pictures to make
a diagnosis, or start treatment. -
9:01 - 9:02Mother cells.
-
9:02 - 9:06Did you know that the biggest store
of mother cells is in our skin? -
9:06 - 9:08In the fat of the skin.
-
9:09 - 9:11But now, I'd like to ponder,
-
9:11 - 9:14because we are talking
about advances, the Internet, -
9:14 - 9:16mother cells, high technology.
-
9:16 - 9:19And we cannot forget
something very important: -
9:19 - 9:21the patient.
-
9:22 - 9:28Sometimes, doctors treat the illness
very well, but not the patient. -
9:29 - 9:31When talking about health systems,

9:31 - 9:34either public, or private,
concerted, or all of them, -
9:34 - 9:37the best health system
-
9:37 - 9:40should be based on the patient.
-
9:40 - 9:42The real personalized medicine
-
9:42 - 9:44is to be sitting down with the patient
-
9:44 - 9:48listening to them, touching them
and being involved in their problems. -
9:48 - 9:51I remember a boss who always told me,
-
9:51 - 9:54"Every time a patient is coming
with a very wide psoriasis -
9:54 - 9:55or some sort of skin illness,
-
9:55 - 9:58you must sit down with the patient
and touch them without gloves -
9:58 - 10:01to show them
that you're not rejecting them. -
10:01 - 10:03Because in medicine,
as in many other fields -
10:03 - 10:06there is much information
-
10:06 - 10:08but little wisdom.
-
10:08 - 10:12It's time to come back
to the roots of medicine. -
10:12 - 10:13Which means,
-
10:13 - 10:19what the Mayo brothers said
when they set up the Mayo Clinic, -
10:19 - 10:22"Necessities of patients are the first".
-
10:25 - 10:30As you can see, there is a fascinating
relationship between skin and mind. -
10:30 - 10:32Many skin patients
-
10:32 - 10:34suffer from depression
or anxiety due to illness, -
10:34 - 10:36but it also can happen vice versa.
-
10:36 - 10:38There are many changes,
-
10:38 - 10:40for example, our skin gets red
when we are nervous, -
10:40 - 10:43our hair falls out often
when we are anxious, -
10:43 - 10:44or dermatitis is worsening.
-
10:44 - 10:45Or psoriasis.
-
10:45 - 10:49Did you know that brain cells
and skin cells -
10:49 - 10:52came from the same embrionary cells?
-
10:52 - 10:54This is something very important.
-
10:54 - 10:59because our skin is not just
a cover on our body, but an organ. -
10:59 - 11:01The biggest organ that we have.
-
11:01 - 11:05It is an active organ
in contact with all our being, -
11:05 - 11:07and it is an organ which speaks to us,
-
11:07 - 11:11telling us what is happening inside
physically and mentally. -
11:12 - 11:15One of the most difficult talks
-
11:15 - 11:20was a few months ago, when I had to talk
at my four-year old daughter's school. -
11:20 - 11:25The talk was about being a doctor,
and being a dermatologist. -
11:25 - 11:28I started by asking them a question,
-
11:28 - 11:30"What is most cool about being a doctor?"
-
11:30 - 11:34A boy quickly got up and said,
"You can see people naked" -
11:34 - 11:35(Laughter)
-
11:36 - 11:40And then a girl said,
"You can help lots of people." -
11:41 - 11:43And it's true, the best thing
about being a doctor -
11:43 - 11:45is that you can help lots of people
-
11:45 - 11:47because although a cure
is not always possible, -
11:47 - 11:49at least we can mitigate pain
-
11:49 - 11:50and as Nietzche said,
-
11:50 - 11:53"The doctor who cannot understand a soul
-
11:53 - 11:55wil never understand a body."
-
11:56 - 11:58So, two messages
for everybody to take home -
11:58 - 12:00from today's talk:
-
12:00 - 12:01First,
-
12:01 - 12:04We can change people's lives
throughh their skin -
12:04 - 12:05and second,
-
12:05 - 12:07the doctor who only knows about medicine
-
12:07 - 12:09doesn't know anything.

12:09 - 12:12I would like to finish with this case:
-
12:12 - 12:14my mother.
-
12:14 - 12:17Here she is with my
six month old daughter. -
12:17 - 12:19It was 4 years ago.
-
12:19 - 12:23She was diagnosed with lung cancer,
-
12:23 - 12:25and breast cancer, a year later.
-
12:25 - 12:27So she has received during this time
-
12:27 - 12:30chemotherapy, radiotherapy, etc.
-
12:32 - 12:34You might not believe it,
-
12:35 - 12:41but the biggest concern
for my mother was losing her hair; -
12:41 - 12:43a secondary effect of the treatment.
-
12:44 - 12:47It can sound frivolous
but around 90% cancer patients -
12:47 - 12:50are very worried about
losing hair and eyebrows. -
12:50 - 12:53Modern dermatology
can help these patients a lot; -
12:54 - 12:59controlling the secondary effects
from chemo and radio, -
12:59 - 13:01over hair, skin and nails.

13:02 - 13:05So, I would like to finish
saying that finally my mother -
13:05 - 13:09is not bothered anymore by the fact
that her son became a dermatologist, -
13:09 - 13:11because she has learned
that there is nothing deeper, -
13:11 - 13:12than our own skin.
