Myomectomy - Applying the tonic

0:04 - 0:08Before starting the myomectomy procedure, the

0:08 - 0:12surgeon has to pull up the bowel

0:12 - 0:16with the sterile pack

0:16 - 0:19so that the bowel will be retracted upwards

0:19 - 0:25and to facilitate proper and easier visualization

0:25 - 0:27of the surgical field.

0:27 - 0:33After proper preparation of retraction of the bowel and

0:33 - 0:41then insertion of the retractor to widen the surgical field

0:43 - 0:49then the next step is to apply tonically?

0:49 - 0:54because the most important step in a myomectomy is to conduct the surgery with
-
0:54 - 0:56less blood loss
-
0:56 - 1:02thus, hemostatis or decreasing hemhorrage is a very important step in myomectomy
-
1:02 - 1:05because myomectomy is going to be a blind x unless you
-
1:05 - 1:10conduct in a way which decrease blood loss
-
1:10 - 1:12the interoperative blood loss might be very high.

1:12 - 1:19There are different techniques but the common method which we use is to apply
-
1:19 - 1:24tonicity, which is to use a #8 catheter.

1:24 - 1:26So we open a window on both sides of
-
1:26 - 1:28lateral aspect of the utuerus, then tightly
-
1:28 - 1:38close it or suture it, or wrapping the catheter
-
1:38 - 1:41so that catheter is going to occlude the
-
1:41 - 1:47the uterine arteries on both sides and then decrease the blood flow to the upper part of the uterus
-
1:47 - 1:49thereby decrease the blood loss.

1:49 - 1:53So after applying a tonicy on the lower aspect of the utuerus

1:53 - 1:59sometimes, depending on the need, the size of the myoma, the tonicy ?
-
1:59 - 2:03also on the ovarian vessels might be needed
-
2:03 - 2:07but usually tonicy applied on the uterine vessels is usually sufficient
-
2:07 - 2:13Currently one can apply or inject a vassopressin
-
2:13 - 2:20which is a kind of drug which will decrease the blood loss to the myoma
-
2:20 - 2:24and then you can conduct the surgery.
