Caesarean Section: Step 6 - Closure of Abdomen

0:01 - 0:09Now we have reached the stage of closing the abdomen

0:09 - 0:12we don't close the p pereotneum

0:12 - 0:15which heals by itself in a few days

0:15 - 0:25in some cases you will see that some surgeons will approximate the rectus abdominus muscle
-
0:25 - 0:29but that not also necessary
-
0:29 - 0:34as you can see now the first step of closing the abdomen is closing the rectus sheath
-
0:34 - 0:37so the rectus sheath or the fascia will be closed by usually
-
0:37 - 0:40Vicryl
-
0:40 - 0:47and the first step would be to start from the angles and we are going to suture
-
0:47 - 0:53it in a continuous nonlocking sutures
-
0:53 - 0:58and when we are closing the fascia is to make sure is that
-
0:58 - 1:09the firstly that the knot we use at the first site where we are going to do the knot at the angle should be
-
1:09 - 1:16locked very well, and the knot should be tight and locked at least four to five times
-
1:16 - 1:24and then when we are doing the continuous suture we should at least in each margin
-
1:24 - 1:33go inside 1.5 cm inside the rectus sheath so that we avoid any risk
-
1:33 - 1:39of tearing of the surture and subsequent dehiscence of the wound
-
1:39 - 1:49so we should make sure we should grasp enough of the rectus sheathh at least 1.5 cm from the margin of the wound
-
1:49 - 3:06while we are doing the continuous suture
-
3:06 - 3:11so once we reach the other end of the wound we should make sure just like we did
-
3:11 - 3:15at the closure of the facia at the first end
-
3:15 - 3:25we should make sure the knot is strong and the knot is repeated at least four or five times
-
3:25 - 3:31and then the next step after we are finished with this
-
3:31 - 3:36we should make sure there are no bleeders inside the subcutaneous fat
-
3:36 - 3:43and occasionally if it is a very deep subcutaneous fat we may do a some
-
3:43 - 3:55interrupted stitch in the subcutaneous fat to avoid a big dead space that could result in subsequent infection
-
3:55 - 4:03but if it is not a deep one then we don't need to put any stitches in the subcutaneous fat
-
4:03 - 4:05and then in the final stage of the procedure
-
4:05 - 4:08is closing the skin
-
4:08 - 4:11so the skin would be usually closed by a
-
4:11 - 4:17subcuticular stitch that you can see now
-
4:17 - 4:23using a 2 O or a 3 O stitch
-
4:23 - 6:09it will be a subcuticular continuous stitch that we are going to apply
-
6:09 - 6:14So after we are done with the closure of the skin
-
6:14 - 6:16before we dress the wound we have to clean it
-
6:16 - 6:26again with some povodone iodine then we put a sterile dressing
-
6:26 - 6:30and tape the wound
-
6:30 - 6:34and after we are done with this

6:34 - 6:37we also have to check and clean the vagina
-
6:37 - 6:41of the woman for any clots, accumulating clots
-
6:41 - 6:45so we'd have to do this with assistance
