The neuroscience of psychedelic drugs, music and nostalgia

    I've studied two fundamental
    human experiences

    that have taught me
    an awful lot about emotion
  • 0:10 - 0:14
    and that may hold the keys
    to a revolution in psychiatry.
  • 0:14 - 0:16
    The first is how we experience music.
  • 0:16 - 0:19
    The second is how we experience
    psychedelic drugs
  • 0:19 - 0:22
    such as LSD and magic mushrooms,
  • 0:22 - 0:23
    or psilocybin,
  • 0:23 - 0:26
    which is the active component
    in magic mushrooms.
  • 0:26 - 0:30
    You may be wondering what these two things
    have in common outside of Woodstock.
  • 0:30 - 0:34
    After all, music is not
    a physical substance.
  • 0:34 - 0:37
    It can be described as a limited set
    of vibrations in the air
  • 0:37 - 0:39
    that can be detected by your ear.
  • 0:39 - 0:43
    And music may seem to have more to do
    with aesthetics than with biology
  • 0:43 - 0:44
    or chemistry.
  • 0:44 - 0:47
    Psychedelic drugs, on the other hand,
    are physical substances.
  • 0:47 - 0:50
    They are chemical compounds
    that you can ingest
  • 0:50 - 0:52
    that directly interact
    with brain chemistry
  • 0:52 - 0:54
    and change your experience of the world.
  • 0:55 - 0:57
    This change is temporary,
  • 0:57 - 1:00
    but the effects of this change
    can alter the course of your life.
  • 1:01 - 1:02
    But let's face it:
  • 1:02 - 1:03
    psychedelics have the potential
  • 1:03 - 1:06
    to trigger unexpected
    and potentially dangerous effects.
  • 1:06 - 1:10
    So what could these two very different
    things possibly have in common?
  • 1:11 - 1:16
    I've found that music and psychedelics
    can impact our well-being
  • 1:16 - 1:18
    in powerful and complementary ways.
  • 1:19 - 1:21
    Music can have a direct
    impact on our emotions,
  • 1:21 - 1:23
    with measurable impacts in the brain;
  • 1:23 - 1:26
    psychedelic drugs,
    under the right circumstances,
  • 1:26 - 1:28
    may have therapeutic effects.
  • 1:28 - 1:30
    These effects can be manifest in patterns
  • 1:30 - 1:33
    that we can study
    and document with brain scans.
  • 1:34 - 1:37
    And together, and leveraged
    in a purposeful fashion,
  • 1:37 - 1:40
    music and psychedelics may have
    an even greater healing impact
  • 1:40 - 1:41
    on patients.
  • 1:41 - 1:45
    What's more, these effects can be manifest
    in healthier and happier lives
  • 1:45 - 1:47
    and more integrated personalities.
  • 1:48 - 1:51
    I began my journey into the mental
    health benefits of music
  • 1:51 - 1:53
    long before I ever intended
    to make such a journey.
  • 1:54 - 1:56
    For roughly half of my life,
    I've been a musician,
  • 1:56 - 1:58
    having played in community orchestras,
  • 1:58 - 2:00
    community theaters,
  • 2:00 - 2:02
    wedding bands, a salsa-merengue band.
  • 2:02 - 2:06
    I was a member of a string band
    in Philadelphia for many years.
  • 2:06 - 2:08
    And for the better part
    of my formative years,
  • 2:08 - 2:12
    I was the drummer
    in a Weezer-Nirvana cover band
  • 2:12 - 2:14
    that morphed into a hardcore punk band.
  • 2:14 - 2:15
    (Laughter)
  • 2:15 - 2:16
    That's right.
  • 2:16 - 2:18
    Drummer in a punk band.
  • 2:18 - 2:22
    But it wasn't until I really began
    my career in psychology and neuroscience
  • 2:22 - 2:27
    that I began to also appreciate
    how widely and how deeply we as a species,
  • 2:28 - 2:30
    both implicitly and explicitly,
  • 2:30 - 2:33
    use music as a tool to try
    to regulate our emotions
  • 2:33 - 2:34
    and to heal.
  • 2:34 - 2:37
    And for some of us, music keeps us going.
  • 2:37 - 2:39
    For others, music isn't quite enough.
  • 2:40 - 2:42
    For me, this led to some
    fascinating questions.
  • 2:42 - 2:46
    I began to use music as a tool
    to study emotion and memory in the brain.
  • 2:46 - 2:50
    My first scientific study was focused
    on music-evoked nostalgia.
  • 2:50 - 2:52
    Nostalgia's a rich and bittersweet emotion
  • 2:52 - 2:55
    that is intimately tied up
    with our autobiographical memories.
  • 2:56 - 3:00
    We can often encounter nostalgia
    in unexpected places.
  • 3:00 - 3:03
    You may have had the experience
    of driving down the highway,
  • 3:03 - 3:04
    turning on the radio
  • 3:04 - 3:07
    or firing up your favorite music
    recommendation service,
  • 3:07 - 3:09
    and you hear a song
    you haven't heard in ages,
  • 3:09 - 3:11
    and you get immediately
    transported back in time
  • 3:11 - 3:14
    and dumped into this immersive memory --
  • 3:14 - 3:16
    something you haven't
    thought about in ages
  • 3:16 - 3:17
    but was very meaningful to you --
  • 3:17 - 3:19
    maybe wedding day or senior prom
  • 3:19 - 3:21
    or the birth of your first child
  • 3:21 - 3:22
    or the death of a loved one.
  • 3:23 - 3:26
    Music can serve as a powerful context cue
  • 3:26 - 3:31
    for deeply meaningful and intensely vivid
    nostalgic memories such as these.
  • 3:32 - 3:36
    Nostalgia, in a sense,
    is deeply woven into our sense of self.
  • 3:37 - 3:39
    Who are we at our most authentic selves?
  • 3:39 - 3:41
    By connecting us
    with our emotional histories,
  • 3:41 - 3:45
    nostalgia can help us
    to stave off sadness, loneliness,
  • 3:45 - 3:46
    existential threat
  • 3:46 - 3:47
    and even the imminence of death
  • 3:47 - 3:50
    and the approaching horizon
    of our lives as we age.
  • 3:51 - 3:55
    To try to get a better understanding
    of how music may tap into nostalgia
  • 3:55 - 3:57
    and what that may be doing in the brain,
  • 3:57 - 4:01
    I began to work with computational
    models of music cognition.
  • 4:01 - 4:04
    I applied these models
    to interrogate brain activity
  • 4:04 - 4:07
    that was recorded
    while people were listening
  • 4:07 - 4:11
    to nostalgia-evoking
    and nonnostalgia-evoking music.
  • 4:11 - 4:14
    And importantly,
    at least to a brain geek like me,
  • 4:14 - 4:18
    I found that nostalgia was able
    to recruit a wide network of brain regions
  • 4:18 - 4:21
    involved in multiple levels
    of different cognitive processes.
  • 4:21 - 4:24
    Whereas nonnostalgic music
    could recruit brain regions
  • 4:24 - 4:25
    such as Heschl's gyrus,
  • 4:25 - 4:28
    involved in basic auditory processing,
  • 4:28 - 4:29
    or Broca's area,
  • 4:29 - 4:31
    which is involved in processing
    grammar and syntax
  • 4:31 - 4:34
    not only in language but also in music,
  • 4:34 - 4:37
    nostalgia was able to recruit
    these brain regions and more.
  • 4:37 - 4:40
    Brain regions such as the substantia nigra
    involved in reward processing
  • 4:40 - 4:44
    or the anterior insula involved
    in the visceral experience of emotion
  • 4:44 - 4:47
    or brain regions
    in the inferior frontal gyrus
  • 4:47 - 4:49
    that are involved
    in autobiographical memories.
  • 4:49 - 4:52
    Nostalgia was also able to recruit
    a wide network of brain regions
  • 4:52 - 4:55
    in prefrontal, frontal, cingulate,
    insular, parietal, occipital
  • 4:56 - 4:57
    and subcortical brain regions
  • 4:57 - 5:00
    that span nearly all
    of our cognitive faculties.
  • 5:00 - 5:03
    This may explain why nostalgia
    can have such an outsized impact on us.
  • 5:04 - 5:05
    But as powerful as it is in the moment,
  • 5:05 - 5:09
    the salve of music-evoked
    nostalgia eventually fades.
  • 5:09 - 5:12
    Nostalgia may be more of a Band-Aid,
  • 5:12 - 5:13
    less of an antibiotic
  • 5:13 - 5:17
    and typically far from a surgical
    intervention for our emotional health.
  • 5:18 - 5:20
    Music can draw out nostalgia
  • 5:20 - 5:22
    and music and nostalgia
    can move our feelings,
  • 5:22 - 5:24
    but how do we make these feelings stick?
  • 5:25 - 5:26
    After studying the nostalgic brain,
  • 5:26 - 5:28
    I joined a team
    at Johns Hopkins University
  • 5:28 - 5:31
    that was studying the effects
    of psychedelic drugs,
  • 5:31 - 5:35
    and I quickly began to learn how deeply
    a piece of music could impact a person
  • 5:35 - 5:36
    during a psychedelic experience.
  • 5:36 - 5:39
    I was previously vexed by the difficulty
    in predicting precisely
  • 5:39 - 5:42
    what musical stimulus would evoke
    precisely what response
  • 5:42 - 5:44
    within a given individual.
  • 5:44 - 5:48
    A song that evokes nostalgia in one person
    could just as easily evoke disinterest
  • 5:48 - 5:49
    or disgust in another person.
  • 5:49 - 5:54
    I began to learn how deeply most music
    seemed to impact most people
  • 5:54 - 5:56
    during psychedelic experiences.
  • 5:57 - 5:58
    Since at least the late '50s,
  • 5:58 - 6:00
    the value of using music to help people
  • 6:01 - 6:03
    to navigate psychedelic
    experiences was clear.
  • 6:03 - 6:06
    We continue this tradition
    in our modern research,
  • 6:06 - 6:08
    asking volunteers to listen to music
  • 6:08 - 6:11
    during the course
    of a psychedelic therapy session,
  • 6:11 - 6:15
    and despite most people being
    mostly naive to the music that we play
  • 6:15 - 6:17
    before they get into the sessions,
  • 6:17 - 6:18
    after these sessions,
  • 6:18 - 6:21
    our volunteers practically
    beg us for the playlists.
  • 6:21 - 6:24
    And some of them report
    returning to the songs
  • 6:24 - 6:27
    that were most impactful to them
    during their psychedelic experience
  • 6:27 - 6:30
    weeks, months and even many years
    after the experience.
  • 6:30 - 6:34
    Somehow, these songs
    can turn into touchstones
  • 6:34 - 6:40
    that can rekindle the most powerful
    and impactful and insightful experiences
  • 6:40 - 6:43
    that people encountered
    during their psychedelic sessions.
  • 6:44 - 6:46
    Of course, I had to know
    what was going on here.
  • 6:46 - 6:48
    I began to deploy
    my batteries of questionnaires
  • 6:48 - 6:50
    and my carefully crafted experiments
  • 6:50 - 6:51
    and my big, fancy MRI machines
  • 6:51 - 6:54
    to try to determine
    just what could be happening
  • 6:54 - 6:56
    during these experiences
  • 6:56 - 7:00
    that could explain the depth
    of impact that people were encountering.
  • 7:01 - 7:03
    At a basic psychological level,
  • 7:03 - 7:04
    my colleagues and I determined that,
  • 7:04 - 7:07
    for instance, LSD can increase
    positive emotions
  • 7:07 - 7:10
    that are uniquely encountered
    during music listening.
  • 7:10 - 7:14
    This may have relevance just by itself
    for healthy individuals
  • 7:14 - 7:17
    as well as people suffering from mood
    and substance-use disorders.
  • 7:17 - 7:19
    But what was happening in the brain?
  • 7:20 - 7:24
    Earlier we learned that the entire brain
    listens to nostalgic music.
  • 7:25 - 7:28
    When applying computational models
    of music cognition
  • 7:28 - 7:32
    to interrogate brain activity
    that was recorded during music listening
  • 7:32 - 7:34
    under the effects of LSD,
  • 7:34 - 7:38
    we found that the entire brain
    was listening to music
  • 7:38 - 7:40
    and psychedelics were turning up the gain.
  • 7:42 - 7:45
    Where nostalgia could recruit
    brain regions involved in language,
  • 7:45 - 7:46
    memory and emotion,
  • 7:46 - 7:48
    psychedelics were recruiting
    these brain regions
  • 7:48 - 7:50
    at least twice as strongly.
  • 7:50 - 7:52
    Brain regions such as the thalamus,
  • 7:52 - 7:54
    that's involved in basic
    sensory processing
  • 7:54 - 7:55
    or the medial prefrontal cortex
  • 7:56 - 7:57
    and the posterior singular cortex,
  • 7:57 - 8:00
    which can be involved in memory
    and emotion and mental imagery.
  • 8:00 - 8:03
    These brain regions were recruited
    up to four times as strongly
  • 8:03 - 8:06
    during the effects of LSD
    than without LSD.
  • 8:07 - 8:10
    Psychedelics turn the knob up to 11.
  • 8:11 - 8:14
    Sensory information
    is more richly experienced in the brain;
  • 8:14 - 8:17
    emotions, memories
    and mental imagery are supercharged,
  • 8:17 - 8:20
    and it may be the wholesale
    and strong recruitment
  • 8:20 - 8:24
    of a wide range of brain regions
    during these experiences
  • 8:24 - 8:26
    that is the necessary key
    to unlocking change
  • 8:26 - 8:30
    that sets these drugs
    and these experiences apart from others.
  • 8:31 - 8:33
    And the effects can be long-lasting.
  • 8:33 - 8:35
    In a study of healthy individuals,
  • 8:35 - 8:38
    I demonstrated that a single
    high dose of psilocybin
  • 8:38 - 8:42
    could reduce negative affect in volunteers
    for at least a week after psilocybin,
  • 8:42 - 8:43
    and increase positive affect
  • 8:43 - 8:46
    for at least a month
    after a single high dose of psilocybin.
  • 8:47 - 8:48
    The reduction in negative affect
  • 8:48 - 8:51
    that we observed after
    psilocybin administration
  • 8:51 - 8:54
    was accompanied by a reduction,
    one week after psilocybin,
  • 8:54 - 8:57
    in the response of a primitive
    brain region called the amygdala
  • 8:57 - 8:59
    to emotional stimuli.
  • 8:59 - 9:03
    In a separate study in patients
    with major depressive disorder,
  • 9:03 - 9:07
    not only did we observe a substantial
    decrease in depression severity
  • 9:07 - 9:11
    in most of our patients
    after two doses of psilocybin,
  • 9:11 - 9:15
    but we also observed a reduction
    in the amygdala response
  • 9:15 - 9:18
    to negative affective
    stimuli, specifically,
  • 9:18 - 9:20
    one week after psilocybin.
  • 9:20 - 9:22
    This reduction in amygdala response
  • 9:22 - 9:25
    was associated with an enduring
    reduction in depression severity
  • 9:25 - 9:28
    for at least three months
    after psilocybin administration,
  • 9:28 - 9:30
    but frankly, we're still counting.
  • 9:31 - 9:33
    So what does this all mean?
  • 9:34 - 9:38
    It means that music
    and psychedelics may be able
  • 9:38 - 9:42
    to alter the entire brain
    for a period of time,
  • 9:42 - 9:46
    and that may lead to a change
    in neural circuitry
  • 9:46 - 9:49
    that may be stuck in patterns
    of negative emotional bias.
  • 9:50 - 9:53
    This may be able to give people
    a period of relief
  • 9:53 - 9:56
    from the grip and the claws
    of negative emotion.
  • 9:57 - 10:00
    And that may be just enough to give
    someone access to new perspectives
  • 10:00 - 10:02
    on their selves and their lives
  • 10:02 - 10:05
    and begin on the road to healing
    from years of depression.
  • 10:06 - 10:09
    These drugs are early
    in stages of research,
  • 10:09 - 10:14
    but they're now being researched
    for a wide range of medical indications.
  • 10:14 - 10:15
    There's evidence growing
  • 10:15 - 10:18
    that psychedelics may be effective
    in helping to treat mood disorders
  • 10:18 - 10:20
    such as major depressive disorder,
  • 10:20 - 10:21
    treatment-resistant depression
  • 10:21 - 10:23
    and the depression and anxiety
  • 10:23 - 10:25
    that accompany a late-stage
    cancer diagnosis.
  • 10:25 - 10:28
    There's also evidence accumulating
    that psychedelics may be effective
  • 10:28 - 10:31
    in helping to treat a wide range
    of substance-use disorders,
  • 10:31 - 10:33
    including smoking, drinking
    and cocaine use.
  • 10:33 - 10:35
    Additional studies
    are either being planned
  • 10:35 - 10:37
    or are already underway
  • 10:37 - 10:40
    to determine whether psychedelics
    may be effective in treating
  • 10:40 - 10:42
    an even wider range
    of intractable disorders
  • 10:42 - 10:45
    such as OCD, PTSD,
  • 10:45 - 10:47
    opioid-use disorder and anorexia.
  • 10:48 - 10:51
    At this point it might be reasonable
    to take a step back
  • 10:51 - 10:54
    and say, "Are psychedelics
    being sold as a panacea?"
  • 10:54 - 10:56
    And if so, we should
    be rightfully skeptical.
  • 10:56 - 11:00
    Why should we expect such a small family
    of compounds to be so effective
  • 11:00 - 11:03
    in treating such a wide range
    of disparate disorders?
  • 11:05 - 11:07
    Here's a perspective we might consider.
  • 11:09 - 11:11
    Some of these disorders
    share a common thread.
  • 11:12 - 11:15
    At some level,
  • 11:15 - 11:18
    mood disorders and substance-use disorders
    involve negative affect
  • 11:18 - 11:20
    and a disconnection
    from our most authentic selves.
  • 11:22 - 11:24
    Psychedelics may break that mold.
  • 11:25 - 11:28
    Psychedelics and music
    may represent a one-two punch
  • 11:28 - 11:33
    that can operate on psychological
    neural processes such as negative affect
  • 11:33 - 11:35
    that cut across and contribute
    to multiple disorders.
  • 11:35 - 11:40
    It may be that targeting
    such transdiagnostic processes
  • 11:40 - 11:43
    is what's necessary to really help people
  • 11:43 - 11:47
    to develop the resources
    that they need to begin to recover
  • 11:47 - 11:49
    from years of depression
    and substance use.
  • 11:50 - 11:53
    They say you never get a second chance
    to make a first impression,
  • 11:54 - 11:56
    and that may be true
    for psychedelic drugs.
  • 11:56 - 11:58
    After all, no matter
    how much data come out
  • 11:58 - 12:02
    for the potential of therapeutic
    effects of these drugs,
  • 12:02 - 12:05
    there are still some who are stuck
    on the stigma from the '60s and '70s:
  • 12:05 - 12:08
    myths of the wildly addictive
    properties of these drugs
  • 12:08 - 12:10
    or myths of genetic abnormalities
  • 12:10 - 12:12
    or birth defects after
    being exposed to these drugs,
  • 12:12 - 12:15
    or fears that people
    are going to lose their minds
  • 12:15 - 12:16
    and go insane --
  • 12:16 - 12:17
    or maybe even most pervasive
  • 12:17 - 12:20
    is the sense that these effects
    are necessarily real
  • 12:20 - 12:25
    and that they're a necessary outcome
    of having been exposed to these compounds.
  • 12:25 - 12:28
    It may be time to change
    our thinking on that point.
  • 12:30 - 12:33
    No one should expect psychedelic
    drugs to work for everyone.
  • 12:33 - 12:36
    No one should expect psychedelic
    drugs to work for everything.
  • 12:36 - 12:37
    They're powerful compounds
  • 12:37 - 12:42
    that need to be administered
    under carefully controlled circumstances.
  • 12:42 - 12:45
    And there are almost certainly
    people in this world
  • 12:45 - 12:47
    for whom psychedelics
    are incredibly dangerous.
  • 12:48 - 12:49
    But ...
  • 12:50 - 12:54
    antibiotics administered to the wrong
    person under the wrong conditions
  • 12:54 - 12:56
    can be incredibly dangerous, if not worse.
  • 12:57 - 13:00
    But administered to the right person
    under the right conditions,
  • 13:00 - 13:01
    antibiotics save lives.
  • 13:02 - 13:06
    Administered to the right people
    under the right conditions,
  • 13:06 - 13:08
    psychedelic drugs may save lives.
  • 13:11 - 13:16
    It can often feel like it's impossible
    to heal our hearts and our minds
  • 13:16 - 13:17
    and to grow,
  • 13:17 - 13:21
    but I truly believe that we all have
    the resources within ourselves
  • 13:21 - 13:22
    to do just that.
  • 13:22 - 13:26
    The challenge is often identifying
    and connecting with those resources,
  • 13:26 - 13:29
    and it may be that psychedelics
    and music can help people
  • 13:29 - 13:31
    to do just that.
  • 13:32 - 13:37
    Together, psychedelics and music
    may be able to open our minds to change
  • 13:37 - 13:38
    and direct that change,
  • 13:38 - 13:42
    reconnect us with our most
    authentic selves
  • 13:42 - 13:44
    and allow us access to the things
  • 13:44 - 13:47
    that really allow us to make
    meaning in this world
  • 13:47 - 13:49
    and reconnect
  • 13:49 - 13:50
    with our most authentic selves.
