what[br]the instrument looks like. 0:00:49.245,0:00:53.453 Before administering the MoCA, you wanna[br]make sure you have a patient's full 0:00:53.453,0:00:56.002 attention, that you're[br]speaking clearly and 0:00:56.002,0:01:00.292 in a reasonable enough volume so that[br]hearing confounds do not become an issue. 0:01:00.292,0:01:04.947 Make sure before you start that you[br]record the date, the patient's age, and 0:01:04.947,0:01:09.674 level of education at the top of the form[br]because you'll need that information 0:01:09.674,0:01:11.623 later when it comes to scoring. 0:01:11.623,0:01:15.258 We'll talk a little bit about[br]administration and scoring of the MoCA. 0:01:15.258,0:01:18.079 The MoCA is one of the most sensitive, but 0:01:18.079,0:01:22.841 also one of the more technical[br]screening instruments to administer. 0:01:22.841,0:01:27.270 The instructions are very nuanced, and[br]so it's important to read through them 0:01:27.270,0:01:31.519 carefully before actually trying to[br]administer the screen with a patient. 0:01:31.519,0:01:34.544 You'll need to know which[br]items can be repeated, and 0:01:34.544,0:01:39.267 how questions should be phrased, because[br]this is important for some of the items. 0:01:39.267,0:01:43.400 Again, the MoCA instructions[br]are very clear and very exacting. 0:01:43.400,0:01:48.246 And therefore, I'm only going to[br]highlight a few administration issues on 0:01:48.246,0:01:49.465 some of the items. 0:01:49.465,0:01:52.330 Question number one is about trail making. 0:01:52.330,0:01:57.690 That is, the patient has to alternate back[br]and forth between numbers and letters. 0:01:57.690,0:02:02.860 Note that if a patient self-corrects[br]an error, and recognizes the error, 0:02:02.860,0:02:06.431 and changes course,[br]you can give them full credit. 0:02:06.431,0:02:11.206 However, uncorrected mistakes of[br]any kind are scored zero points. 0:02:13.435,0:02:14.606 Question number three is[br]a clock drawing task. 0:02:14.606,0:02:19.845 It's important to keep in mind[br]here that scoring of this clock is 0:02:19.845,0:02:26.360 broken up into three separate components,[br]each of which is awarded one point. 0:02:26.360,0:02:31.288 Give patients the benefit of the doubt for[br]the contour or the shape of the circle. 0:02:31.288,0:02:36.261 This is because in clinical experience,[br]when name more than eleven, and[br]so receives a full credit, one point. 0:08:03.866,0:08:07.525 And for abstraction, for the first item, 0:08:07.525,0:08:11.700 train and bicycle,[br]Sam struggled a little bit. 0:08:11.700,0:08:17.384 His answer related to wheels that[br]operate in the same way or have elbows. 0:08:17.384,0:08:20.393 And that's considered[br]a more concrete response. 0:08:20.393,0:08:25.270 The MoCA instructions detail out[br]a little bit more specifically 0:08:25.270,0:08:28.433 what is a good response or a bad response. 0:08:28.433,0:08:32.826 For that item, the train and bicycle,[br]Sam did not not receive full credit. 0:08:32.826,0:08:36.580 But he did answer appropriately when asked[br]about the similarity between a watch 0:08:36.580,0:08:37.205 and a ruler. 0:08:37.205,0:08:41.634 So he receives one out of a total[br]two points in that direction. 0:08:41.634,0:08:45.668 When Sam was asked to remember the words[br]that were given to him earlier, 0:08:45.668,0:08:49.366 unfortunately he was not able to[br]remember any of the words, and so 0:08:49.366,0:08:50.725 received zero points. 0:08:50.725,0:08:54.654 For the last section orientation,[br]Sam was able to answer all of those 0:08:54.654,0:08:57.649 questions correctly, and[br]so received six points. 0:08:57.649,0:09:01.490 When all of the sub components[br]are added
