00:03:19.108 --> 00:03:22.244 so that's gad generalized anxiety disorder 00:03:23.135 --> 00:03:26.906 and as with all of the things that i talk about 00:03:26.906 --> 00:03:30.322 i mean anxiety disorders have at least like i don't know 00:03:30.337 --> 00:03:34.916 like 50 pages in the dsm so this is a very succinct version 00:03:34.916 --> 00:03:37.887 now the next thing i wanna talk about is social phobia 00:03:37.887 --> 00:03:40.164 now many of you talk about having social anxiety 00:03:40.164 --> 00:03:42.958 and from what i can read in the dsm 00:03:42.958 --> 00:03:46.059 social anxieyt isn't actually a diagnosis 00:03:46.059 --> 00:03:49.431 under the anxiety disorders it would be called social phobia 00:03:49.431 --> 00:03:52.915 and they say that the essential feature of social phobia 00:03:52.915 --> 00:03:56.485 is a marked and persistent fear of social or performance 00:03:56.485 --> 00:04:00.842 situations in which embarrassment may occur 00:04:00.842 --> 00:04:05.013 now i find this to be most prevalent with my teen clients 00:04:05.013 --> 00:04:08.283 and my young adult clients the ones in college and stuff 00:04:08.283 --> 00:04:11.854 because we're in social situations a lot and it can be very 00:04:11.854 --> 00:04:14.265 like we may be in a new high school and we're already 00:04:14.265 --> 00:04:16.592 nervous and so then we start to worry about what 00:04:16.592 --> 00:04:18.093 everybody thinks and we don't want to be embarrassed 00:04:18.093 --> 00:04:19.293 and we don't want to embarrass ourselves 00:04:19.293 --> 00:04:22.430 and oh that person's giggling uh they're giggling about me 00:04:22.430 --> 00:04:24.767 and that's kind of how this presents itself 00:04:24.767 --> 00:04:28.701 we think that a lot of times anybody that's giggling 00:04:28.701 --> 00:04:32.473 or looking or somebody is pointing we automatically 00:04:32.473 --> 00:04:34.992 think that they're talking about us we're doing something 00:04:34.992 --> 00:04:37.748 embarrassing and it's terrible so that's kind of what 00:04:37.748 --> 00:04:40.948 social phobia is and under social phobia it says 00:04:40.948 --> 00:04:45.220 social anxiety disorder so that's kind of 00:04:45.220 --> 00:04:50.193 where that falls and that is something that i honestly 00:04:50.193 --> 00:04:54.618 along with all the anxiety disorders it's really important 00:04:54.618 --> 00:04:57.631 that we go to therapy and we talk about this with someone 00:04:57.631 --> 00:05:00.403 and we kind of process it through and in my experience 00:05:00.403 --> 00:05:03.837 CBT cognitive behavioral therapy is the best with this 00:05:03.837 --> 00:05:07.543 because we'll do what we call downward i think it's actually 00:05:07.543 --> 00:05:10.248 technically downward arrow questioning where 00:05:10.248 --> 00:05:15.081 we try to logically talk ourselves out of these you know 00:05:15.081 --> 00:05:18.487 kind of anxiety provoking situations like okay well 00:05:18.487 --> 00:05:22.914 there are 700 people in the cafeteria today 00:05:22.914 --> 00:05:24.760 what are the chances of that person laughing 00:05:24.760 --> 00:05:28.996 is laughing about us, well 1 in 700, well is that really 00:05:28.996 --> 00:05:33.414 you know a high risk situation? is it most likely that they're 00:05:33.414 --> 00:05:36.672 thinking and laughing about me or is it less likely that they're 00:05:36.672 --> 00:05:39.275 thinking and laughing at me? well it's probably less likely 00:05:39.275 --> 00:05:42.177 and we kind of talk ourselves out of it 00:05:42.177 --> 00:05:46.114 so if you suffer from social anxiety or social phobia 00:05:46.114 --> 00:05:47.831 and you're wondering what to do about it 00:05:47.831 --> 00:05:50.484 i would look into seeing a CBT therapist okay? 00:05:50.484 --> 00:05:53.787 so that's just a little tip and then 00:05:53.787 --> 00:05:57.090 what is agoraphobia everybody talks about that too, right? 00:05:57.090 --> 00:05:59.862 and that falls under this as well now agoraphobia 00:05:59.862 --> 00:06:04.733 differs from social anxiety or social phobia in that 00:06:04.733 --> 00:06:07.868 it doesn't have to do with social situations 00:06:07.868 --> 00:06:11.873 it's not relational it's not like well that girl is pointing at me 00:06:11.873 --> 00:06:15.270 and she's being really mean or that guy was whispering 00:06:15.270 --> 00:06:18.579 and i know he was whispering about me it's not relational 00:06:18.579 --> 00:06:23.517 with those kinds of people it is all to do with us getting into 00:06:23.517 --> 00:06:27.915 a situation which usually is social or is just out of our house 00:06:27.915 --> 00:06:30.525 out of our comfort zone so we may be safe at home 00:06:30.525 --> 00:06:33.995 we may be safe at work anything else not safe 00:06:33.995 --> 00:06:38.200 and our main concern is that we'll get somewhere and 00:06:38.200 --> 00:06:41.330 we can't leave either we can't leave easily 00:06:41.330 --> 00:06:44.465 or without embarrassment and that's our biggest worry 00:06:44.465 --> 00:06:47.776 it's like i'm gonna get somewhere and i'm gonna start to 00:06:47.776 --> 00:06:50.277 feel overwhelmed and then i won't be able to leave 00:06:50.277 --> 00:06:53.715 or i'll have to embarrass myself in front of people to leave 00:06:53.715 --> 00:06:56.717 because i'll be stuck you know like i'm in the middle seat 00:06:56.717 --> 00:06:59.998 in this opera and i'm gonna have to get up and disturb 00:06:59.998 --> 00:07:01.997 all these people it's gonna be really embarrassing for me 00:07:01.997 --> 00:07:05.427 so that's kind of more what agoraphobia is versus 00:07:05.427 --> 00:07:07.997 social anxiety so social anxiety is more relational 00:07:07.997 --> 00:07:11.799 agoraphobia is just like situational if that makes sense 00:07:11.799 --> 00:07:14.470 that's kind of in my mind how if i had my white board 00:07:14.470 --> 00:07:17.897 i would draw a line and i would break them down like that 00:07:17.897 --> 00:07:23.331 so that is that then there are panic attacks and panic disorder 00:07:23.331 --> 00:07:25.756 now panic attacks i know many of you have said that 00:07:25.756 --> 00:07:28.616 you've had them and that you have them a lot 00:07:28.616 --> 00:07:31.625 and they're really overwhelming and that's the truth 00:07:31.625 --> 00:07:36.223 they really suck and they're terrible and they not only 00:07:36.223 --> 00:07:37.958 come on quickly but once we have them 00:07:37.958 --> 00:07:39.359 then we always worry that we're going to 00:07:39.359 --> 00:07:45.867 have more of them and the symptoms of panic attacks 00:07:45.867 --> 00:07:49.270 can be they say somatic or cognitive in nature so it can be 00:07:49.270 --> 00:07:52.674 like heart palpitations i can be sweating i can be trembling 00:07:52.674 --> 00:07:55.778 i can be shaking the most common thing i hear 00:07:55.778 --> 00:07:59.415 in my practice is i feel like i'm drowning and i think that's 00:07:59.415 --> 00:08:02.119 because we have those heart palpitations and we 00:08:02.119 --> 00:08:05.663 don't breathe very well and we feel like we're drowning 00:08:05.663 --> 00:08:10.530 and so these will happen usually in ten minutes or less 00:08:10.530 --> 00:08:14.998 and you can have a panic attack but only if 00:08:14.998 --> 00:08:16.864 and i want to make sure i say this correctly 00:08:16.864 --> 00:08:21.503 only if we have recurrent and unexpected panic attacks 00:08:21.503 --> 00:08:23.665 followed by at least a month of worry that we're going to 00:08:23.665 --> 00:08:27.738 have another do we have panic disorder okay? 00:08:27.738 --> 00:08:29.413 so that's how those differentiate because 00:08:29.413 --> 00:08:31.278 panic attacks can happen and some of us with 00:08:31.278 --> 00:08:34.182 generalized anxiety disorder may have a panic attack 00:08:34.182 --> 00:08:36.163 every once in a while when we're put in a really 00:08:36.163 --> 00:08:40.756 stressful situation but unless we have them recurrent 00:08:40.756 --> 00:08:43.924 and we worry about them all the time we don't have 00:08:43.924 --> 00:08:46.998 panic disorder we would have generalized anxiety disorder 00:08:46.998 --> 00:08:50.764 or agoraphobia with panic attacks okay? so that's how 00:08:50.764 --> 00:08:55.639 those are differentiated and just to give you an idea 00:08:55.639 --> 00:08:58.503 of how many other things are included under anxiety 00:08:58.503 --> 00:09:01.642 disorders we have panic disorder without agoraphobia 00:09:01.642 --> 00:09:04.280 panic disorder with agoraphobia so you can see how 00:09:04.280 --> 00:09:06.747 all of these can be attached on to one another 00:09:06.747 --> 00:09:10.685 with or without so that's why i just gave you you know 00:09:10.685 --> 00:09:13.421 those most common and what i hear most from you and 00:09:13.421 --> 00:09:17.269 what you wanted me to talk about so that is 00:09:17.269 --> 00:09:21.750 an umbrella view in the most common panic related 00:09:21.750 --> 00:09:26.267 and anxiety related disorders and different attacks 00:09:26.267 --> 00:09:30.839 that we can have okay?
