0:00:00.343,0:00:02.509 When I was asked to - [br]invited, rather, 0:00:02.515,0:00:05.283 to give this talk [br]a couple of months ago 0:00:05.291,0:00:08.742 we discussed a number [br]of titles with the organizers 0:00:08.742,0:00:12.291 and a lot of different items were [br]kicked around and were discussed, 0:00:12.291,0:00:15.697 but nobody suggested[br]this one that you see here today 0:00:15.743,0:00:18.575 and the reason for that was,[br]two months ago 0:00:18.575,0:00:21.845 ebola was escalating exponentially 0:00:21.845,0:00:25.410 and spreading over wider geographic areas[br]than we had ever seen 0:00:25.410,0:00:29.237 and the world was terrified, [br]concerned and alarmed 0:00:29.237,0:00:34.075 by this disease, in a way we've not [br]seen in recent history. 0:00:34.083,0:00:38.172 But today, I can stand here [br]and I can talk to you 0:00:38.172,0:00:40.501 about beating ebola, 0:00:40.501,0:00:43.712 because of people [br]whom you've never heard of, 0:00:43.712,0:00:46.906 people like Peter Clement, 0:00:48.786,0:00:53.956 a Liberian doctor,[br]who's working in Lofa county, 0:00:54.102,0:00:59.619 a place that many of you have [br]never heard of, probably, in Liberia. 0:01:00.736,0:01:03.435 The reason that Lofa county [br]is so important 0:01:03.435,0:01:06.218 is because [br]about five months ago, 0:01:06.218,0:01:09.741 when the epidemic was [br]just starting to escalate, 0:01:09.741,0:01:14.596 Lofa county was right at the center, [br]the epicenter of this epidemic. 0:01:14.596,0:01:16.832 At that time, MSF [br](Doctors Without Borders) 0:01:16.832,0:01:18.698 and the treatment center there, 0:01:18.698,0:01:21.304 they were seeing dozens of patients [br]every single day, 0:01:21.304,0:01:25.646 and these patients, these communities[br]were becoming more and more terrified 0:01:25.646,0:01:30.042 as time went by, with this disease [br]and what it was doing to their families, 0:01:30.042,0:01:33.729 to the communities, [br]to the children, to the relatives. 0:01:33.729,0:01:38.607 And so Peter Clement was charged with [br]driving that 12 hour long rough road, 0:01:38.607,0:01:42.106 from Monrovia, the capital,[br]up to Lofa county, 0:01:42.106,0:01:47.451 to try and help bring control[br]to the escalating epidemic there. 0:01:47.451,0:01:52.927 And what Peter found when he arrived[br]was a terror that I just mentioned to you. 0:01:52.927,0:01:56.470 So he sat down with the local chiefs, [br]and he listened. 0:01:57.045,0:02:01.065 And what he heard was heartbreaking. 0:02:01.065,0:02:05.094 He heard about the devastation [br]and the desperation 0:02:05.094,0:02:08.017 of people affected by this disease. 0:02:08.017,0:02:10.764 He heard the heartbreaking stories about 0:02:10.764,0:02:13.607 not just the damage that [br]ebola did to people, 0:02:13.607,0:02:16.946 but what it did to families [br]and what it did to communities. 0:02:18.156,0:02:22.119 And he listened to the chiefs, [br]the local chiefs there 0:02:22.119,0:02:23.748 and what they told him - they said 0:02:23.748,0:02:26.528 "When our children are sick, [br]when our children are dying, 0:02:26.528,0:02:29.797 we can't hold them at a time when [br]we want to be closest to them. 0:02:29.797,0:02:34.812 When our relatives die, we can't [br]take care of them as our tradition demands 0:02:34.812,0:02:37.381 we are not allowed to wash [br]the bodies to bury them 0:02:37.381,0:02:40.401 the way our communities and [br]our rituals demand. 0:02:40.401,0:02:43.920 And for this reason, [br]they were deeply disturbed, deeply alarmed 0:02:43.920,0:02:47.630 and the entire epidemic [br]was unraveling in front of them. 0:02:47.630,0:02:50.804 People were turning on the health care [br]workers who had come, 0:02:50.804,0:02:54.281 the heroes who come to try and help save [br]and the community 0:02:54.281,0:02:58.245 to help work with the community,[br]And they were unable to access them. 0:02:58.734,0:03:05.247 And what happened then was [br]Peter explained to the leaders, 0:03:05.247,0:03:07.908 the leaders listened. [br]They turned the tables. 0:03:07.933,0:03:11.764 And Peter explained what ebola was. [br]He explained what the disease was, 0:03:11.764,0:03:14.013 he explained what it did[br]to their communities. 0:03:14.013,0:03:18.865 And he explained that ebola threatened [br]everything that made us human. 0:03:18.865,0:03:23.337 Ebola means you can't hold your children[br]the way you would in this situation. 0:03:23.337,0:03:25.791 You can't bury your dead [br]the way that you would. 0:03:25.791,0:03:30.222 You have to trust these people [br]in the space suits to do that for you. 0:03:30.222,0:03:33.591 And ladies and gentlemen, what [br]happened then was rather extraordinary; 0:03:33.591,0:03:37.218 the community, health workers, Peter, [br]they sat down together 0:03:37.218,0:03:41.717 and they put together a new plan [br]for controlling ebola in that Lofa county. 0:03:41.717,0:03:46.208 And the reason that this is such [br]an important story, ladies and gentlemen, 0:03:46.208,0:03:51.672 is because today, this County, which is [br]right at the center of this epidemic 0:03:51.672,0:03:54.667 you've been watching, [br]you've been seeing on the newspapers, 0:03:54.667,0:03:58.330 you've been seeing on [br]the television screens, 0:03:58.330,0:04:04.116 tody, loafer County is nearly eight weeks [br]without seeing a single case of ebola. 0:04:04.116,0:04:11.203 (Applause) 0:04:11.203,0:04:14.314 Now this doesn't mean that [br]the job is done obviously. 0:04:14.314,0:04:17.636 There still are huge risk that [br]there will be additional cases there. 0:04:17.636,0:04:21.411 But what it does teach us is that [br]ebola can be beaten. 0:04:21.411,0:04:22.710 That's the key thing. 0:04:22.710,0:04:26.313 Even on the scale, even with the rapid [br]kind of growth that we saw 0:04:26.313,0:04:30.999 in this environment here, [br]we now know ebola can be beaten. 0:04:30.999,0:04:35.238 When communities come together [br]with health care workers work together, 0:04:35.238,0:04:37.581 that's when this disease can be stopped. 0:04:37.581,0:04:41.266 But how did ebola end up in Lofa county [br]in the first place? 0:04:41.266,0:04:46.146 Well, for that, we have to go back [br]12 months, to the start of this epidemic. 0:04:46.146,0:04:50.009 And as many you know, [br]this virus went undetected, 0:04:50.009,0:04:53.530 evaded the detection for 3 or 4 months[br]when it began. 0:04:53.839,0:04:56.439 That's because this is not [br]a disease of West Africa, 0:04:56.439,0:04:59.650 it's a disease of Central Africa, [br]half a continent away. 0:04:59.650,0:05:01.549 People hadn't seen the disease before, 0:05:01.549,0:05:03.916 health workers hadn't seen [br]the disease before -- 0:05:03.916,0:05:06.176 they didn't know what [br]they were dealing with, 0:05:06.176,0:05:08.099 and they make it [br]even more complicated, 0:05:08.099,0:05:12.549 the virus itself was causing a symptom,[br]is a type of a presentation 0:05:12.549,0:05:15.147 that wasn't classical of the disease. 0:05:15.147,0:05:19.391 So people didn't even recognize [br]a disease, people who knew ebola 0:05:19.391,0:05:23.168 For that reason it evaded a detection [br]for some time, 0:05:23.168,0:05:26.328 But contrary to public belief [br]sometimes these days, 0:05:26.355,0:05:32.341 once the virus was detected, [br]there was a rapid surge in up support. 0:05:32.341,0:05:35.641 MSF rep relief set up [br]the ebola treatment center 0:05:35.641,0:05:38.907 as many of you know, in the area[br]the World Health Organization 0:05:38.907,0:05:41.136 and the partners that worked [br]with deplored 0:05:41.136,0:05:43.986 and eventually hundreds of people [br]over the next two months 0:05:43.986,0:05:46.660 to be able to help track the virus. 0:05:46.660,0:05:50.174 The problem is, ladies and gentlemen,[br]is by then, this virus, 0:05:50.174,0:05:53.318 well known now as ebola, [br]had spread too far. 0:05:53.318,0:05:57.218 It had already outstripped what was [br]one of the largest responses 0:05:57.218,0:06:00.588 that had been mounted so far[br]toward ebola outbreak. 0:06:00.588,0:06:03.478 By the middle of the year,[br]not just Guinea 0:06:03.478,0:06:07.329 but now Sierra Leone and Liberia[br]were also infected. 0:06:07.329,0:06:11.632 And the virus was spreading geographically[br]the numbers were increasing 0:06:11.632,0:06:16.943 and at this time, not only were [br]hundreds of people infected 0:06:16.943,0:06:19.833 and dying of the disease,[br]but as importantly, 0:06:19.834,0:06:21.847 the front line responders, 0:06:21.847,0:06:24.688 the people who to try and help the people, 0:06:24.688,0:06:27.834 the health care workers, the other [br]responders 0:06:27.834,0:06:30.950 were also sick and dying by the dozens. 0:06:30.950,0:06:33.927 The presidents of these countries [br]recognized emergencies 0:06:33.927,0:06:37.224 they met right around that time, [br]they agreed on common action 0:06:37.229,0:06:40.986 and they put together an emergency [br]joint operation center in Conakry 0:06:40.986,0:06:45.993 to try and work together to finish this[br]disease and get it stopped, 0:06:45.993,0:06:48.720 to implement the strategies[br]we talked about. 0:06:48.732,0:06:52.792 But what happened then was something [br]we had never seen before with ebola. 0:06:52.843,0:06:56.970 What happened then was the virus, [br]someone sick with the virus, 0:06:56.970,0:06:59.860 boarded an airplane, [br]flew to another country, 0:07:00.022,0:07:04.000 and for the first time,[br]we saw in another distant country 0:07:04.000,0:07:06.720 the virus pop up again. 0:07:06.720,0:07:11.184 This time it was in Nigeria [br]in the teemy metropolis of Legos 0:07:11.184,0:07:12.884 21 million people. 0:07:13.174,0:07:15.964 Now the virus was in that environment. 0:07:16.014,0:07:19.784 And as you can anticipate, [br]there was international alarm, 0:07:19.836,0:07:23.466 international concern on a scale that [br]we haven't seen in recent years 0:07:23.475,0:07:25.865 caused by a disease like this. 0:07:25.915,0:07:30.065 The World Health Organization immediately [br]called together an expert panel, 0:07:30.083,0:07:33.393 looked at the situation, [br]declared an international emergency. 0:07:33.481,0:07:39.235 And in doing so, the expectations would be[br]that there be a huge outpouring of 0:07:39.235,0:07:43.665 international assistance to help these [br]countries which were in so much trouble 0:07:43.687,0:07:46.729 and concerns at that time. 0:07:46.729,0:07:48.972 But what we saw was [br]something very different. 0:07:48.972,0:07:53.042 There was some great response. 0:07:53.060,0:07:57.402 A number of countries came to assist,[br]many many NGOs and others as you know, 0:07:57.402,0:08:00.812 but at the same time, the opposite [br]happened in many places. 0:08:00.812,0:08:06.902 Alarm through -- alarm escalated and[br]very soon these countries found themselves 0:08:06.984,0:08:11.194 not receiving the support they needed[br]but increasingly isolated. 0:08:11.257,0:08:15.687 What we saw with commercial airlines[br]started flying into these countries 0:08:16.099,0:08:19.059 and people who hadn't even been[br]exposed to the virus 0:08:19.087,0:08:21.213 were no longer allowed to travel. 0:08:21.213,0:08:24.944 This cause not only problems, obviously, [br]for the countries themselves, 0:08:24.944,0:08:26.714 but also for the response. 0:08:26.995,0:08:29.880 Those organizations that we're trying to[br]bring people in, 0:08:29.880,0:08:32.033 to try and help them[br]respond to the outbreak 0:08:32.033,0:08:34.104 they could not get [br]people on airplanes, 0:08:34.104,0:08:37.355 they couldn't get in to the countries[br]to be able to respond. 0:08:37.355,0:08:39.736 In that situation,[br]ladies and gentleman, 0:08:39.764,0:08:43.384 a virus like ebola takes advantage. 0:08:43.405,0:08:47.726 And what we saw then was something [br]also we hadn't seen before. 0:08:47.744,0:08:50.440 Not only did this virus [br]continue in the places 0:08:50.440,0:08:54.450 where they'd already become infected,[br]but then it started to escalate 0:08:54.499,0:08:56.779 and we saw the case numbers that you [br]see here, 0:08:56.854,0:08:59.804 something we never seen before [br]on such a scale 0:08:59.821,0:09:02.631 and exponential increase ebola cases 0:09:02.663,0:09:06.963 not just in these countries or the areas [br]already infected in these countries 0:09:06.963,0:09:11.043 but also spreading further and [br]deeper into these countries. 0:09:11.088,0:09:13.275 Ladies and gentleman, this was one of the 0:09:13.275,0:09:18.845 most concerning international emergencies [br]in public health we've ever seen. 0:09:18.995,0:09:22.535 And what happened in these countries, and [br]many of you saw, again, 0:09:22.535,0:09:24.955 on the televisions, read about in [br]the newspapers, 0:09:25.257,0:09:30.787 we saw the health system start to collapse[br]under the weight of this epidemic. 0:09:30.879,0:09:35.699 We saw the schools begin to close, [br]markets no longer started, 0:09:35.736,0:09:39.286 no longer functioned the way [br]that they should in these countries. 0:09:39.397,0:09:43.397 We saw the misinformation and [br]the misperceptions started to spread 0:09:43.423,0:09:46.751 even faster through the communities [br]which became even more alarmed 0:09:46.751,0:09:48.091 about the situation. 0:09:48.091,0:09:51.561 They started to recoil from those people [br]that you saw in the space suits, 0:09:51.561,0:09:54.141 as they call them, [br]would come to help them. 0:09:54.153,0:09:57.323 And then the situation deteriorated [br]even further. 0:09:57.371,0:10:00.031 The countries had to declare a state of[br]emergency. 0:10:00.056,0:10:03.116 Large populations need to [br]be quarantined in some areas 0:10:03.116,0:10:08.986 and then riots broke out. [br]It was a very very terrifying situation 0:10:09.034,0:10:12.094 And the world many people began to ask 0:10:12.094,0:10:15.714 can we ever stop ebola [br]when it starts to spread like this 0:10:15.755,0:10:19.755 and they started to ask how well [br]do we really know this virus. 0:10:20.512,0:10:23.472 The reality is we don't know [br]ebola extremely well. 0:10:23.505,0:10:27.555 It's a relatively modern disease [br]in terms of what we know about it 0:10:27.562,0:10:30.862 we've known the disease only for [br]forty years since it first popped up 0:10:30.872,0:10:33.995 in Central Africa, 1976. 0:10:33.995,0:10:37.445 But despite that we do know many things, 0:10:37.479,0:10:40.689 we know that this virus probably survives[br]in a type of a bat, 0:10:40.784,0:10:44.204 we know that it probably enters [br]a human population 0:10:44.209,0:10:47.129 when we come in contact with[br]the wild animal that has been 0:10:47.135,0:10:50.495 infected with the virus [br]and probably sickened by it. 0:10:50.538,0:10:53.518 Then we know that the[br]virus spreads from person to person 0:10:53.534,0:10:55.964 through contaminated body fluids. 0:10:55.988,0:10:58.575 And as you've all seen we know [br]the horrific disease at 0:10:58.575,0:11:02.495 it then causes in humans[br]where we see this disease cause 0:11:02.558,0:11:05.285 severe fevers, diarrhea, vomiting, [br]and then 0:11:05.285,0:11:10.945 unfortunately, in 70% of the cases [br]or probably more, death. 0:11:11.487,0:11:16.797 This is a very dangerous, debilitating, [br]and deadly disease. 0:11:17.116,0:11:21.476 But despite the fact that we've not known[br]this disease for a particularly long time, 0:11:21.476,0:11:25.806 and we don't know everything about it,[br]we do know how to stop this disease. 0:11:25.861,0:11:29.070 This four things that are critical to [br]stopping ebola. 0:11:29.070,0:11:33.940 First and foremost, the communities [br]have got to understand this disease, 0:11:33.991,0:11:37.600 they've got to understand [br]how it spreads and how to stop it. 0:11:37.600,0:11:41.607 And then we've got to be able to have [br]a system that could find every single case 0:11:41.607,0:11:45.527 every contact of those cases and [br]begin to track to transmission chains 0:11:45.527,0:11:47.957 so that you can stop transmission. 0:11:47.957,0:11:51.387 We have to have treatment center[br]specialized ebola treatment centers, 0:11:51.461,0:11:56.471 where the workers can be protected[br]as they try to provide support 0:11:56.495,0:11:58.624 to these add to the people [br]who are infected, 0:11:58.624,0:12:01.213 so that they might survive the disease 0:12:01.213,0:12:04.834 and then for those who do die, [br]we have to ensure 0:12:04.850,0:12:08.930 there is a safe, but at the same time, [br]dignified burial process, 0:12:08.963,0:12:12.299 so that there is no spread [br]at that time as well. 0:12:12.299,0:12:17.929 So we do know how to stop ebola and these[br]strategies work, ladies and gentlemen, 0:12:17.955,0:12:22.085 the virus was stopped in Nigeria [br]by these four strategies 0:12:22.143,0:12:24.600 and the people implementing them [br]obviously, 0:12:24.600,0:12:28.380 it was stopped in Senegal where it had[br]spread, and also in the other countries 0:12:28.380,0:12:32.160 that were affected by this virus, [br]in this outbreak. 0:12:32.160,0:12:36.346 So there's no question that [br]these strategies actually work. 0:12:36.346,0:12:41.529 The big question, ladies and gentlemen,[br]was whether these strategies could work 0:12:41.558,0:12:45.598 on this scale, in this situation, with [br]so many countries affected with 0:12:45.625,0:12:49.051 the kinda exponential growth [br]that you saw. 0:12:49.051,0:12:53.981 That was a big question that we were [br]facing just two for three months ago. 0:12:54.096,0:12:58.096 Today, we know [br]the answer to that question. 0:12:58.120,0:13:01.516 We know that answer because of [br]the extraordinary work 0:13:01.516,0:13:05.545 of incredible group of NGOs and [br]governments, of local leaders, 0:13:05.545,0:13:10.155 of UN agencies and many humanitarian [br]and other organizations 0:13:10.197,0:13:14.457 that came and joined the fight, [br]try and stop ebola in West Africa. 0:13:14.629,0:13:17.669 But what had to be done there [br]was slightly different. 0:13:17.680,0:13:21.010 These countries took those strategies [br]I just showed you; 0:13:21.028,0:13:25.078 the communities and community engagement[br]the case finding, contact tracing, etc. 0:13:25.115,0:13:27.955 and they turn them on their head. 0:13:27.988,0:13:30.778 There was so much disease [br]they approached it differently. 0:13:30.778,0:13:36.208 What they decided to do was they would [br]first try and slow down this epidemic 0:13:36.296,0:13:40.646 by rapidly building as many beds [br]as possible so that they specialized 0:13:40.676,0:13:44.669 treatment centers so that they could [br]control, they could prevent the disease 0:13:44.669,0:13:46.433 from spreading from those were infected. 0:13:46.433,0:13:49.237 They would rapidly build out[br]many many burial teams so that 0:13:49.237,0:13:51.201 they could safely [br]deal with the dead. 0:13:51.201,0:13:54.499 With that, they were trying to slow[br]this outbreakts see if it could 0:13:54.499,0:13:57.925 actually then be controlled using [br]the classic approach 0:13:57.925,0:14:00.341 of case finding in contact tracing. 0:14:00.341,0:14:04.998 And when I went to West Africa about [br]three months ago when I was there, 0:14:04.998,0:14:07.358 what I saw was extraordinary 0:14:07.370,0:14:11.770 I saw presidents opening emergency[br]operation centers themselves against ebola 0:14:11.790,0:14:16.110 so that they could personally coordinate [br]and oversee in champion 0:14:16.153,0:14:19.573 this surge of international support [br]to try and stop this disease. 0:14:19.593,0:14:23.604 We saw military's from within [br]those countries in from far beyond 0:14:23.604,0:14:26.137 coming in to help build [br]ebola treatment centers 0:14:26.137,0:14:29.210 that could be used to isolate [br]those who are sick. 0:14:34.158,0:14:37.343 to help train the community [br]so that they could actually 0:14:29.210,0:14:34.158 We saw the Red Cross movement working with[br]its partner agencies on the ground there 0:14:37.343,0:14:41.683 safely bury their dead[br]in a dignified manner themselves, 0:14:41.882,0:14:46.282 and we saw the UN agencies the World Food [br]Programme build a tremendous 0:14:46.354,0:14:50.329 air bridge that could get responders [br]to every single corner at these countries 0:14:50.329,0:14:53.899 rapidly to be able to implement [br]the strategies that we just talked about. 0:14:54.022,0:14:57.922 What we saw, ladies and gentlemen, [br]which is probably most impressive 0:14:57.965,0:15:02.025 was just incredible work by the government[br]by the leaders in these countries 0:15:02.108,0:15:07.176 with the communities to try insure people[br]understood this disease, 0:15:07.206,0:15:11.786 understood the extraordinary things they [br]would have to do to try and stop ebola. 0:15:11.859,0:15:16.869 And as a result, ladies and gentlemen,[br]we saw something that we did not know 0:15:16.898,0:15:21.138 only two or three months earlier[br]whether or not it would be possible. 0:15:21.175,0:15:25.034 What we saw was what you see now [br]in this graph, 0:15:25.034,0:15:27.333 when we took stock [br]on the first of December. 0:15:27.333,0:15:30.907 what we saw was we could bend that curve, [br]so to speak, 0:15:30.907,0:15:34.373 change this exponential growth[br]and bring some hope back 0:15:34.373,0:15:37.003 to the ability to control this outbreak. 0:15:37.003,0:15:39.013 And for this reason, ladies and gentlemen, 0:15:39.021,0:15:43.451 there's absolutely no question now [br]that we can catch up with this outbreak 0:15:43.475,0:15:46.715 in West Africa, and we can beat ebola. 0:15:47.480,0:15:51.482 The big question though is that [br]many people are asking even when 0:15:51.482,0:15:53.519 they saw this curve, they said, [br]"hang on a minute, that's great 0:15:53.519,0:15:57.457 you can slow it down, but can you [br]actually drive it down to zero?" 0:15:57.457,0:16:01.443 Now we already answered that question [br]at right back the beginning of this talk 0:16:01.443,0:16:07.603 when I spoke about Lofa county [br]in Liberia, 0:16:07.625,0:16:12.063 we told you the story haw Lofa county [br]got to a situation where they have 0:16:12.063,0:16:14.896 not seen ebola for eight weeks.[br]But this similar stories 0:16:14.896,0:16:19.566 from the other countries as well, [br]from GuĂŠckĂŠdou in Guinea, 0:16:19.566,0:16:23.930 the first area where the first case was[br]actually diagnosed. 0:16:23.947,0:16:28.374 We've seen very very few cases[br]in the last couple of months, 0:16:28.374,0:16:33.175 and here in Kenema, in Sierra Leone, [br]another area in the epicenter, 0:16:33.175,0:16:36.885 we have not seen the virus for more than [br]a couple of weeks -- 0:16:36.886,0:16:41.386 way too early to declare victory obviously[br]but evidence, ladies and gentlemen, 0:16:41.429,0:16:46.129 not only can the response catch up [br]to the disease but this disease 0:16:46.133,0:16:50.633 can be driven to zero. The challenge now, [br]of course, is doing this 0:16:50.633,0:16:55.743 on the scale needed right across these [br]three countries, 0:16:55.761,0:17:00.535 and that is a huge challenge. Because [br]when you've been at something 0:17:00.535,0:17:06.545 for this long, on this scale, two other [br]big threats come in to join the virus. 0:17:06.573,0:17:11.843 The first of those is complacency. [br]The risk that as this disease 0:17:11.851,0:17:15.931 curve starts to bend, the media look [br]elsewhere, the world looks elsewhere. 0:17:16.155,0:17:19.565 Complacency always a risk. [br]And the other risk of course is 0:17:19.586,0:17:23.887 when you've been working so hard [br]for so long and slept so few hours 0:17:23.887,0:17:28.937 over the past months, people are tired, [br]people become fatigued and these new risks 0:17:28.947,0:17:34.327 start to creep into the response. [br]Ladies and gentlemen, I can tell you today 0:17:34.359,0:17:38.509 I've just come back from West Africa.
