0:00:00.000,0:00:01.039 Hey, everybody. 0:00:01.039,0:00:03.800 Today, we’re gonna talk about cyclothymia. 0:00:04.060,0:00:06.320 What is this weird word?[br]What does it mean? 0:00:13.674,0:00:16.463 So like I said, today, we’re gonna talk[br]about cyclothymia. 0:00:16.858,0:00:19.382 What is cyclothymia?[br]Why is that such a strange word? 0:00:19.687,0:00:23.665 Well, once I get talking to you about it,[br]you’ll understand why it sounds like 0:00:23.665,0:00:30.773 “cycle”, “cyclothymia” . So, cyclothymia[br]falls under the bipolar disorder— 0:00:31.745,0:00:35.969 or falls into the bipolar-disorder bucket.[br]I like a bucket. That sounds better. 0:00:35.969,0:00:39.630 So it falls into that category. And I’ve[br]heard from many of you that you struggle 0:00:39.630,0:00:41.840 with this, and you wanted me to talk[br]about it a little bit more. 0:00:41.840,0:00:46.582 So what I’m going to go over today is[br]number one, what is cyclothymia? 0:00:46.772,0:00:51.733 How do we diagnose it? And then, what are[br]the medication and treatment options 0:00:51.850,0:00:52.651 that we have? 0:00:53.012,0:00:55.207 And that’s it. We get in and get out. 0:00:55.560,0:00:57.239 Are you ready? Are you with me? 0:00:57.239,0:00:58.347 Let’s get started. 0:00:58.347,0:01:03.791 Cyclothymia must occur for at least two[br]years in adults, one year in children 0:01:03.791,0:01:07.504 and adolescents. So, yes, if you’re an[br]adolescent, you can still be diagnosed 0:01:07.613,0:01:13.997 with cyclothymia. Now, the reason that they[br]call it cyclothymia—if you remember my old 0:01:14.250,0:01:18.430 video where I talked about bipolar[br]disorder on a whiteboard like a teacher, 0:01:18.430,0:01:23.078 like follow along as I tell you why this[br]is called this, that will make it— 0:01:23.078,0:01:26.408 if you haven’t watched that, watch that,[br]’cause that will make this so 0:01:26.408,0:01:27.668 much more clear. 0:01:27.988,0:01:33.197 So, cyclothymia, there must have been[br]numerous periods of hypomanic episodes, 0:01:33.197,0:01:35.401 meeting the criteria for hypomania. 0:01:35.592,0:01:39.801 If you wonder what hypomania is, come[br]watch my bipolar II video, where I explain 0:01:39.968,0:01:41.448 what that is. 0:01:41.448,0:01:44.700 So here is hypomania. We’re on that[br]whiteboard again. 0:01:44.700,0:01:48.358 So hypomania is here; mania will be up[br]here. 0:01:48.358,0:01:52.158 Then, we’re kind of in the middle, like[br]“Hey, just hanging out”, and then, we get 0:01:52.158,0:01:54.838 into kind of dysthymia. I have a video[br]about that as well. 0:01:54.965,0:01:58.154 I have a lot of videos. If you have[br]questions, just search the word I’m 0:01:58.154,0:02:00.964 talking about, Kati Morton, and it will[br]probably come up. 0:02:00.985,0:02:03.675 So I have a video about dysthymia, and[br]then down here on the bottom, we have 0:02:03.675,0:02:08.945 a major depressive episode, which I also[br]explain in my bipolar II video. 0:02:09.388,0:02:15.418 So, with cyclothymia, we don’t reach[br]mania: we only reach hypomania. 0:02:15.761,0:02:20.043 So, we’ve hit it, but we don’t go above.[br]We just stay here. 0:02:20.072,0:02:22.142 And then depression is all the way down[br]here, MDD. 0:02:22.245,0:02:26.685 We don’t hit that either. We hit[br]dysthymia, that low-grade depression 0:02:26.685,0:02:30.005 that can last for a long time, hence,[br]why it says this must last for at least 0:02:30.005,0:02:31.544 two years. 0:02:32.314,0:02:33.275 Got it? 0:02:33.515,0:02:39.000 So we fall in this kind of middle yet[br]still uncomfortable realm of highs and 0:02:39.000,0:02:39.910 lows. 0:02:40.546,0:02:42.826 Never major, major highs or[br]major, major lows, 0:02:42.850,0:02:47.230 we kind of flow in and out.[br]And that’s also why this goes undiagnosed 0:02:47.241,0:02:51.721 all the time, because for a lot of people,[br]it’s “somewhat” manageable. 0:02:52.053,0:02:56.203 And I use quotes because it’s still[br]not good, but we can kind of tolerate it, 0:02:56.794,0:02:57.564 okay? 0:02:57.564,0:02:59.284 So that’s how we diagnose it. 0:02:59.299,0:03:03.283 I think it’s important to note you don’t[br]meet the criteria for MDD but you meet 0:03:03.283,0:03:08.103 the full criteria for hypomania, okay? [br]So we’re going from hypomania to dysthymia. 0:03:08.584,0:03:11.849 And I want to also note, and I’m just[br]reading from what I wrote down here, 0:03:11.849,0:03:15.279 that if you also meet the criteria for[br]borderline personality disorder, 0:03:15.290,0:03:19.290 that’s fine. You can have both.[br]They can happen simultaneously. 0:03:19.450,0:03:23.374 And this is often comorbid, meaning[br]happening at the same time, with drug and 0:03:23.374,0:03:28.474 alcohol abuse, because like I said, people[br]“tolerate” it okay. They do “okay”, 0:03:28.573,0:03:32.394 but they usually self-medicate. And so[br]it’s often that we find people with 0:03:32.394,0:03:36.226 cyclothymia struggle also with alcohol and[br]drug abuse, okay? 0:03:36.286,0:03:38.569 So that’s what it is.[br]That’s how long it has to last. 0:03:38.569,0:03:40.603 And that’s kind of what it feels like. 0:03:40.603,0:03:42.713 What are our options?[br]So, okay, we have this. 0:03:42.978,0:03:47.018 So, as all bipolar-disorder things, I’ve[br]talked about medication. 0:03:47.294,0:03:51.029 [hissing], I know. I already feel your[br]anger and frustation that you’ll probably 0:03:51.029,0:03:53.825 leave in the comments about, like, “Why[br]do you always say medication?” 0:03:53.825,0:03:56.475 Medication can be so incredibly helpful. 0:03:57.121,0:04:00.017 It’s not for everybody, but it’s[br]definitely something for people who 0:04:00.017,0:04:02.487 struggle with this can be really, really[br]beneficial. 0:04:02.680,0:04:05.640 And the medications that they recommend—[br]obviously, I’m not a doctor. 0:04:05.907,0:04:10.657 You have to see a doctor for this.[br]But they recommend mood stabilizers. 0:04:11.034,0:04:12.880 Wow, say that five times fast. 0:04:12.880,0:04:15.667 Mood stabilizers or[br]antipsychotic medication. 0:04:15.667,0:04:19.257 Now, the reason that they give you these[br]is because obviously, 0:04:19.257,0:04:22.845 we’re floating in between. We’re all over.[br]We feel up and down and all around. 0:04:22.845,0:04:26.615 And mood stabilizers does just that;[br]it works to stabilize our mood. 0:04:26.733,0:04:30.123 And antipsychotic medications also help[br]with that as well, 0:04:30.197,0:04:34.257 and so talk to your doctor if medication[br]is something that you’re looking into. 0:04:34.273,0:04:36.309 And also, they recommend psychotherapy. 0:04:36.639,0:04:38.219 That’s me. Hey. 0:04:38.569,0:04:42.190 We do therapy. And CBT, they find, to be[br]the best because there are a lot 0:04:42.190,0:04:47.050 of behavioral things that you can do to[br]help yourself better manage the symptoms. 0:04:47.349,0:04:51.799 They obviously recommend psychotherapy and[br]medication together. 0:04:52.138,0:04:57.018 It’s like, with our powers combined, we[br]can rule the world. 0:04:57.051,0:04:59.757 You remember like heart—no one wanted to[br]heart—but it was like water, air— 0:04:59.757,0:05:02.527 Anyway, those are best combined. 0:05:03.125,0:05:06.870 So, see a therapist, and see a[br]psychiatrist so you can get all the 0:05:06.870,0:05:07.860 help you need. 0:05:08.276,0:05:09.546 And like I said, CBT is the best. 0:05:09.724,0:05:14.698 They also recommend—and this is something[br]I don’t do, so don’t ask me to do a video 0:05:14.698,0:05:16.913 on it ’cause this isn’t a type of therapy[br]I do. 0:05:16.913,0:05:22.643 But it’s called IPSRT, interpersonal and[br]social rhythm therapy. 0:05:23.680,0:05:27.060 They say this helps us get into a[br]more-consistent schedule, like a rhythm, 0:05:27.546,0:05:31.405 of sleep, wake, eating, et cetera. And[br]that helps better manage your symptoms 0:05:31.405,0:05:32.495 as well. 0:05:32.758,0:05:34.388 I hope you found this helpful. 0:05:34.688,0:05:36.938 And that’s what cyclothymia is. 0:05:37.180,0:05:39.461 And if you haven’t checked out my other[br]videos about bipolar disorder, 0:05:39.461,0:05:42.321 that will definitely help this make even[br]more sense. 0:05:42.360,0:05:44.600 You’ll have an even-bigger picture of[br]what’s going on. 0:05:44.965,0:05:46.714 And don’t forget to subscribe to this[br]video. 0:05:46.714,0:05:50.510 And if you like these kinds of structured,[br]what-is-it, how-can-we-help-ourselves, 0:05:50.510,0:05:51.656 give us a thumbs up. 0:05:51.656,0:05:52.574 And leave your comments below. 0:05:52.574,0:05:54.494 If you have experience with this—[br]we’re a community. 0:05:54.506,0:05:57.076 We work together. We share our comments[br]and our experiences. 0:05:57.598,0:06:00.178 And that’s wonderful and helpful[br]and amazing. 0:06:00.263,0:06:01.943 And I will see y’all next time. 0:06:03.309,0:06:04.149 —Cut 0:06:04.815,0:06:07.275 Go right here with your hands.[br]How far is the box? 0:06:08.921,0:06:11.091 I actually have to think about it.[br]It’s actually very difficult. 0:06:11.091,0:06:12.861 [laughs] And I don’t want to mess up my[br]hair. 0:06:12.861,0:06:15.651 That’s why the pat goes on top, so you[br]have to think about it again to 0:06:15.651,0:06:19.651 — [inaudible][br]No, no, it’s too much. I gotta focus 0:06:19.651,0:06:24.231 on cyclothymia. Stop trying to trick me.[br][whispers] tricky, tricky.
