What is the most difficult symptom to manage?

  • 0:14 - 0:17
What is the most difficult symptom
    to manage?
  • 0:20 - 0:24
    I would say seizures
  • 0:24 - 0:26
    being the first,
  • 0:26 - 0:28
    and the saliva.
  • 0:28 - 0:30
    Secretions.
  • 0:33 - 0:38
    Ruby had problems with airway
  • 0:38 - 0:41
from when she was born really.
  • 0:41 - 0:43
    Looking back now there was
  • 0:43 - 0:47
    a lot of hypotonia, low
    muscle tone in her throat.
  • 0:47 - 0:51
That was a personal thing to
    Ruby, I think.
  • 0:51 - 0:52
    She had airway collapses.
  • 0:53 - 0:57
    Keeping that free and clear,
    through positioning
  • 0:57 - 1:01
    when she was sick and
    required oxygen
  • 1:01 - 1:02
    that was the hardest thing
    to manage.
  • 1:06 - 1:10
    The first very difficult symptom
    to manage
  • 1:10 - 1:13
    were Amelie's seizures.
  • 1:13 - 1:15
    She started having seizures soon
  • 1:15 - 1:17
    after she was PEG fed.
  • 1:17 - 1:21
They weren't that bad but obviously
  • 1:21 - 1:23
it is very difficult to see your child
  • 1:23 - 1:25
    having a seizure, especially
    for the first time.
  • 1:26 - 1:28
    We realised soon enough that
  • 1:28 - 1:31
because Amelie was a little unwell that
  • 1:31 - 1:35
when she didn't feed she was not
    having seizures.
  • 1:35 - 1:39
As we were observing this, we were
  • 1:39 - 1:43
told to try the ketogenic diet on her
  • 1:43 - 1:45
    which has worked quite well.
  • 1:45 - 1:48
    Actually, Amelie who is now
  • 1:49 - 1:52
    had her first seizure
three and a half years ago
  • 1:52 - 1:55
has got quite a good control of
    her seizures.
  • 1:56 - 1:59
    This helps her a lot,
    in general.
  • 1:59 - 2:03
    When they weren't under control
  • 2:03 - 2:06
    they are quite difficult to
    watch and to manage
  • 2:06 - 2:11
because all you can do is
    hold her and comfort her.
  • 2:12 - 2:14
    But, you can't stop them.
  • 2:15 - 2:21
    It's a very difficult symptom
    to deal with when
  • 2:21 - 2:23
    it first starts.
  • 2:23 - 2:25
    I think with Amelie's case
  • 2:25 - 2:28
    you can use lots of
    different medication
  • 2:28 - 2:30
    and we thought we'd try
    the ketogenic diet
  • 2:30 - 2:31
    purely because she is
PEG fed so it is quite
  • 2:32 - 2:33
easy to to mix up the powders
  • 2:34 - 2:37
and give her the right amount
    in each feed.
  • 2:37 - 2:39
We saw a success using it.
  • 2:39 - 2:41
    Some children use it but
    they don't see any difference
  • 2:41 - 2:43
    in their seizure activity.
  • 2:43 - 2:45
In Amelie's case it went from
  • 2:45 - 2:47
every hour and a half or
    two hours
  • 2:48 - 2:50
    to maybe one a day.
  • 2:50 - 2:53
She doesn't have any of those
    big holding breath seizures
  • 2:53 - 2:55
    anymore which were the
    biggest problem.
  • 2:55 - 2:58
    We put that down purely
    due to the diet.
  • 3:00 - 3:03
    Secretions
  • 3:03 - 3:05
    are the most difficult
  • 3:05 - 3:07
and the one which can change
  • 3:07 - 3:11
    the most, minute by minute,
    hour by hour.
  • 3:11 - 3:19
You have to tweak little things,
    sometimes several times a day.
  • 3:19 - 3:23
    They are never really undercontrol
  • 3:23 - 3:26
    so they are always something
there you have to deal with.
  • 3:29 - 3:31
    He is sick a lot.
  • 3:31 - 3:37
    His breathing, and muscus.
