I am a neurosurgeon,
and I'm here to tell you today
that people like me need your help.
And in a few moments, I will tell you how.
But first, let me start off by telling you
about a patient of mine.
This was a woman in her 50s;
she was in generally good shape,
but she had been in and out
of hospital a few times
due to curative breast cancer treatment.
Now, she had gotten a prolapse
from a cervical disc
giving her radiating pain
of an intense kind out into the right arm.
Looking at her MRI before the consultation
I decided to suggest an operation.
Now, neck operations like this
are standardized and they're quick,
but they carry a certain risk.
You make an incision right here,
and you dissect carefully
past the trachea, the esophagus,
and you try not to cut
into the internal carotid artery.
Then you bring in the microscope,
and you carefully remove the disc
and the prolapse in the nerve root canal
without damaging the cord and the nerve
root lying only millimeters underneath.
A worst-case scenario
is a damage to the cord,
which can result in paralysis
from the neck down.
Explaining this to the patient,
she fell silent.
And after a few moments,
she uttered a few very decisive words
for me and for her.
"Doctor, is this really necessary?"
And you know what I realized
right there and then?
It was not.
In fact, when I get patients
like this woman,
I tend to advise not to operate.
So what made me do it this time?
Well, you see, this prolapse
was so delicate
I could practically see myself
pulling it out of the nerve root canal
before she entered the consultation room.
I have to admit it;
I wanted to operate her.
I'd love to operate her.
Operating like this is, after all,
the most fun part of my job.
I think you can relate to this feeling.
My architect neighbor says
he loves just sit and draw
and design houses.
He'd rather do that all day
than talk to the person,
the client paying for the house,
that might even give him
restrictions on what to do.
But like every architect, every surgeon
needs to look the patient in the eye,
and together with the patient,
they need to decide on what is best
for the person having the operation.
And that might sound easy,
but let's look at some statistics.
The tonsils are the two lumps
in the back of your throat;
they can be removed surgically,
and that's called a tonsillectomy.
This chart shows the operation rate
of tonsillectomies in Norway
in different regions.
What might strike you is that
there is twice the chance that your kid -
because this is for only children -
will get the tonsillectomy
in Finnmark than in Trondheim.
The indication in both
regions are the same.
There should be
no difference but there is.
Here's another chart.
The meniscus helps stabilize the knee
and can be torn or fragmented acutely,
typically during sports like soccer.
What you see here is the operation rate
for this condition,
and you see that the operation rate
in Møre og Romsdal
is five times the operation
rate in Stavanger.
Five times.
How can this be?
Did the soccer players
in Møre og Romsdal play dirtier
than anywhere else in the country?
(Laughter)
Probably not.
I've added some information now.
What you see now is the procedures
performed in public hospitals
are light blue.
The one in private clinics is light green.
There is a lot of activity
in the private clinics in Møre og Romsdal,
isn't there?
What does this indicate?
Possible economic motivation
to treat the patients.
And there's more.
Recent research has shown
that the difference of treatment effect
between regular physical therapy
and operations for the knee,
there is no difference.
Meaning that most of the procedures
performed on the chart I've just shown
could have been avoided,
even in Stavanger.
So what am I trying to tell you here?
Even though
most indications for treatments
in the world are standardized,
there is a lot of unnecessary variation
of treatment decisions,
especially in the Western world.
Some people are not getting
the treatment that they need,
but even a greater portion of you
are being overtreated.
"Doctor, is this really necessary?"
I've only heard that question
once in my career.
My colleagues say they never heard
of these words from a patient.
And to turn it the other way around,
how often do you think
you'll get a "no" from a doctor
if you ask such a question?
Researchers have investigated this,
and they come up with
about the same "no" rate wherever they go,
and that is 30 percent.
Meaning, three out of 10 times
your doctor prescribes
or suggests something
that is completely unnecessary.
And you know what they claim
the reason for this is?
Patient pressure.
In other words, you.
You want something to be done.
A friend of mine came to me
for medical advice.
This is a sporty guy,
he does a lot of cross-country
skiing in the wintertime,
he runs in the summertime,
and this time he had gotten
a bad backache whenever he went jogging,
so much that he had to stop doing it.
I did an examination,
I questioned him thoroughly.
What I found out
is that he probably had a degenerated disc
in the lower part of his spine.
Whenever it got strained, it hurt.
He'd already taken up swimming
instead of jogging.
There was really nothing
to do, so I told him,
"You need to be more selective
when it comes to training.
Some activities are good for you,
some are not."
His reply was,
"I want an MRI on my back."
"Why do you want an MRI?"
"I can get it for free
through my insurance at work."
"Come on," I said -
he was also my friend -
"that's not a real reason."
"Well, I think it's going to be good
to see how bad it looks back there."
"When did you start interpreting
MRI scans," I said.
(Laughter)
"Trust me on this,
you're not going to need the scan."
"Well," he said,
and after a while he continued,
"it could be cancer."
(Laughter)
He got the scan, obviously.
And through his insurance at work
he got to see one of my colleagues at work
telling him about the degenerated disc,
that there was nothing to do,
he should keep on swimming
and quit the jogging.
After a while, I met him
again, and he said,
"At least now I know what this is."
But let me ask you a question.
What if all of you in this room
with the same symptoms had an MRI?
And what if all the people
in Norway had an MRI
due to occasional back pain?
The waiting list for an MRI
would quadruple, maybe even more.
And you would all take the spot
on that list from someone
who really had cancer.
So a good doctor sometimes says no,
but the sensible patient also turns down,
sometimes, an opportunity
to get diagnosed or treated.
"Doctor, is this really necessary?"
I know this can be
a difficult question to ask.
In fact, if you go back 50 years,
this was even considered rude.
If the doctor had decided
what to do with you, that's what you did.
A colleague of mine,
now a general practitioner,
was sent to away
on a tuberculosis sanitarium
as a little girl.
For six months.
It was a terrible trauma for her.
She later found out, as a grown-up,
that her tests on tuberculosis
had been negative all along.
The doctor had sent her away
on nothing but wrong suspicion.
No one had dared or even considered
confronting him about it.
Not even her parents.
Today, the Norwegian health minister
talks about the patients'
healthcare service.
The patient is supposed to get advice
from the doctor about what to do.
This is great progress,
but it also puts
more responsibility on you.
You need to get in the front seat
with your doctor
and start sharing decisions
on where to go.
The next time you're in a doctor's office
I want you to ask,
"Doctor, is this really necessary?"
And you might feel in many patients' case
the answer would be "no."
But an operation could also be justified.
So doctors, what are the risks
attached to this operation?
Well, five to 10 percent of patients
will have worsening of pain symptoms.
One to two percent of patients
will have an infection in the wound
or even a re-hemorrhage
that might end up in a re-operation.
0.5 percent of patients
also experience permanent hoarseness,
and a few, but still a few,
will experience reduced function
in the arm or even legs.
"Doctor, are there other options?"
Yes, rest and physical therapy
over some time might get you
perfectly well.
"And what happens if I don't do anything?"
It's not recommended,
but even then there's a slight chance
that you will get well.
Four questions, simple questions.
Consider them your new toolbox
to help us.
Is this really necessary?
What are the risks?
Are there other options?
And what happens if I don't do anything?
Ask them when your doctor
wants to send you to an MRI,
when he prescribes antibiotics
or suggests an operation.
What we know from research
is that one out of five of you,
20 percent,
will change your opinion on what to do.
And by doing that, you will not only
have made your life a whole lot easier
and probably even better,
but the whole healthcare sector
will have benefited from your decision.

Soy neurocirujano,
y estoy aquí para decirles
que gente como yo necesita su ayuda.
En unos instantes les diré cómo.
Pero primero déjenme que
les cuente algo sobre una paciente.
Era una mujer de unos 50 años,
estaba más o menos en buena forma,
pero llevaba un tiempo
entrando y saliendo del hospital
por un tratamiento de cáncer de mama.
Había sufrido un prolapso
de disco cervical
que le provocaba un dolor
que se extendía hasta el brazo derecho.
Al ver su RM antes de la consulta,
decidí sugerirle que se operara.
Las operaciones de cuello
están estandarizadas y son rápidas,
pero conllevan cierto riesgo.

En el peor de los casos,
se daña la médula
y esto puede ocasionar
parálisis del cuello hacia abajo.
Al explicarle esto,
la paciente se quedó callada.
Y, tras un instante, pronunció unas
palabras decisivas para ambos:
"Doctor, ¿de verdad esto es necesario?".
¿Y saben de qué me di cuenta
en ese mismo instante?
De que no lo era.
De hecho, cuando recibo
pacientes como esta mujer,
suelo recomendarles no operarse.
¿Qué me llevó a hacerlo esta vez?
Pues bien, este prolapso era tan delicado,
que podía verme a mí mismo
extrayéndolo del canal del nervio
antes de que ella entrara a la consulta.
Tengo que admitirlo: quería operarla.
Me hubiera encantado operarla.
Al fin y al cabo, las operaciones así
son lo más divertido de mi trabajo.
Creo que conocen esa sensación.
Mi vecino que es arquitecto
dice que le encanta sentarse a dibujar
y diseñar casas.
Y que preferiría hacer eso
en lugar de hablar con
el cliente que paga la casa,
y que hasta podría ponerle
restricciones sobre qué hacer.
Pero, como todo arquitecto,
todo cirujano debe mirar
a sus pacientes a los ojos
y decidir con él cuál es la mejor solución
para quien va a someterse a la operación.
Y quizá suene fácil, pero veamos
algunos datos estadísticos.
Las amígdalas son dos bultos
en la parte posterior de la garganta.
Se extraen mediante una operación
conocida como amigdalectomía.
Este gráfico muestra
la tasa de amigdalectomías
en varias regiones de Noruega.
Lo que les puede sorprender
es que hay el doble de posibilidades
de que a su hijo,
porque esto es para niños,
se lo someta a una amigdalectomía
en Finnmark que en Trondheim.

Consideratele la vostra

Quali sono i rischi?
Ci sono altre opzioni?
Cosa succede se non faccio niente?
Fatele, quando il vostro medico
vuole farvi fare una risonanza,
quando prescrive antibiotici
o consiglia un intervento.

è che uno su cinque tra di voi,
il 20 per cento,
cambierà la propria opinione su cosa fare.

Sou neurocirurgião
e estou aqui hoje para dizer que pessoas
como eu precisam de sua ajuda.
E em alguns instantes, eu direi como.
Mas primeiro, vou começar contando
sobre uma paciente minha.
Ela era uma mulher na faixa dos 50 anos,
em boa forma, de maneira geral,
mas tinha sido hospitalizada algumas vezes
devido ao tratamento que a curou
do câncer de mama.
Agora, ela apresentava uma hérnia de disco
que causava dor intensa
que irradiava para o braço direito.
Examinando a ressonância magnética antes
da consulta, eu decidi sugerir cirurgia.
Cirurgias no pescoço, como essa,
são padronizadas e rápidas,
mas apresentam um certo risco.
Você faz uma incisão bem aqui
e afasta cuidadosamente
a traqueia, o esôfago,
e tenta não cortar
a artéria carótida interna.
Depois você pega o microscópio
e cuidadosamente remove o disco
e o prolapso no canal do nervo
sem danificar a medula e a raiz do nervo,
que estão apenas alguns milímetros abaixo.
A pior das hipóteses é o dano à medula,
que pode resultar em paralisia
do pescoço para baixo.
Quando expliquei à paciente,
ela ficou em silêncio.
E instantes depois,
ela disse algumas poucas palavras
que foram decisivas para mim e para ela.
"Doutor, isso é mesmo necessário?"
E sabem o que percebi naquele momento?
Que não era.
Na verdade, quando atendo
pacientes como essa mulher,
normalmente aconselho a não operar.
Então o que me fez aconselhar
a cirurgia dessa vez?
Bem, essa hérnia era tão delicada
que eu quase podia me ver
tirando-a do canal do nervo
antes da paciente entrar no consultório.
Eu admito, eu queria operá-la.
Eu adoraria operá-la.
Afinal, cirurgias como essa são a parte
mais divertida do meu trabalho.
Acho que vocês conseguem
se identificar com isso.
Meu vizinho arquiteto diz
que ama se sentar para desenhar
e projetar casas.
Ele preferiria fazer isso o dia todo
do que conversar com o cliente
que está pagando pela casa,
o qual pode até impor
restrições sobre o que fazer.
Mas assim como os arquitetos,
todo cirurgião precisa olhar
o paciente nos olhos e juntos
decidirem o que é melhor
para a pessoa a ser operada.
E isso pode soar fácil,
mas vejamos algumas estatísticas.
As amígdalas são dois caroços
na parte de trás da garganta;
elas podem ser removidas cirurgicamente,
isso se chama tonsilectomia.
Esse gráfico mostra a taxa
de tonsilectomias na Noruega,
em diferentes regiões.
O que chama atenção é que há duas
vezes mais chances de que seu filho,
essa é uma cirurgia para crianças,
sofra uma tonsilectomia
em Finnmark do que em Trondheim.
A indicação para cirurgia é a mesma
em ambas as regiões.
Não deveria haver diferença
nas taxas, mas há.
Aqui temos um outro gráfico.
O menisco ajuda a estabilizar o joelho
e pode ser lesionado ou rompido,
geralmente durante a prática
de esportes, como futebol.
Vemos aqui a taxa de operação
para essa lesão,
e uma taxa de cirurgia em Møre og Romsdal
cinco vezes maior do que em Stavanger.
Cinco vezes.
Como isso é possível?
Será que jogadores de futebol
em Møre og Romsdal jogam mais sujo
do que em outras regiões do país?

Provavelmente não.
Adicionei mais informação.
Agora temos os procedimentos
realizados em hospitais públicos,
em azul claro.
Os feitos em hospitais particulares
estão em verde claro.
Há muita atividade nas clínicas privadas
de Møre og Romsdal, não é mesmo?
O que isso indica?
Possivelmente, uma motivação
econômica para tratar os pacientes.
E tem mais.
Pesquisas recentes mostram que não há
diferença entre o resultado do tratamento
com fisioterapia regular
e com cirurgia do joelho.
Ou seja, a maioria dos procedimentos
realizados no gráfico que mostrei
poderia ter sido evitada,
até mesmo em Stavanger.
O que estou tentando dizer?
Apesar da maioria
das indicações para tratamentos
serem padronizadas ao redor do mundo,
há muitas variações desnecessárias
nas decisões de tratamento,
especialmente no Ocidente.
Algumas pessoas não estão recebendo
o tratamento de que precisam,
mas um número ainda maior
está recebendo tratamento excessivo.
"Doutor, isso é mesmo necessário?"
Eu só ouvi essa pergunta uma única vez
durante minha carreira.
Meus colegas dizem nunca terem ouvido
essas palavras de um paciente.
E para mudar a perspectiva,
com que frequência vocês acham
que ouvirão um "não" do médico
se fizerem essa pergunta?
Pesquisadores já investigaram isso
e encontraram, aproximadamente,
a mesma taxa de negativas em toda parte,
uma taxa de 30%.
Ou seja, três em cada dez vezes,
seu médico prescreve ou sugere algo
que é completamente desnecessário.
E sabem qual a razão alegada por eles?
Pressão por parte do paciente.
Em outras palavras, vocês.
Vocês querem que algo seja feito.
Uma vez, um amigo me pediu
aconselhamento médico.
Ele pratica esportes,
faz esqui cross-country no inverno,
corrida no verão,
mas estava sentindo dor nas costas
sempre que fazia jogging,
uma dor tão forte
que ele teve de parar.
Eu o examinei,
questionei minuciosamente.
Descobri que ele
provavelmente tinha uma degeneração
de disco na parte inferior da coluna.
O disco causava dor quando era comprimido.
Ele já tinha trocado jogging por natação.
Não havia muito a ser feito,
então eu disse:
"Você precisa ser mais seletivo
no treinamento.
Algumas atividades são boas para você,
outras, não".
Ele respondeu:
"Quero uma ressonância da minha coluna".
"Por que quer uma ressonância?"
"Eu posso fazê-la de graça
pelo plano de saúde do trabalho."
"Ora, por favor", disse eu;
afinal, era meu amigo.
"Isso não é motivo."
"Acho que seria bom ver como está
a situação nas minhas costas."
"Quando você começou a fazer
diagnóstico de imagens?", perguntei.

"Confie em mim, você
não precisa de um exame."
"Bom", ele disse, e depois completou:
"Pode ser câncer".
(Risos)
Ele fez o exame, claro.
E pelo plano de saúde do trabalho,
se consultou com outro colega meu,
que falou sobre a degeneração de disco,
que não havia nada a ser feito,
que ele deveria continuar
com a natação e parar o jogging.
Depois, nos encontramos
de novo e ele disse:
"Pelo menos, agora eu sei o que é".
Mas vou fazer uma pergunta.
E se todos vocês aqui com os mesmos
sintomas fizessem uma ressonância?
E se todas as pessoas na Noruega
fizessem uma ressonância
devido a dor esporádica nas costas?
A lista de espera para ressonância
aumentaria quatro vezes ou mais.
E vocês tomariam as vagas nessa lista
de pessoas que realmente têm câncer.
Um bom médico às vezes diz não,
mas o paciente sensato também recusa,
às vezes, uma oportunidade
de diagnóstico e tratamento.
"Doutor, isso é mesmo necessário?"
Sei que essa pode ser
uma pergunta difícil de se fazer.
De fato, se voltarmos 50 anos,
isso seria até mesmo considerado rude.
Se o médico tinha decidido o que fazer
com você, você simplesmente fazia.
Uma colega minha,
que agora é clínica geral,
foi enviada para um sanatório
para tuberculosos
quando era pequena.
Por seis meses.
Foi um trauma terrível para ela.
Mas tarde, já adulta, ela descobriu
que seus exames de tuberculose
sempre deram negativo.
O médico a mandou para o sanatório
baseado unicamente em uma falsa suspeita.
Ninguém ousou ou sequer considerou
confrontá-lo a respeito.
Nem mesmo os pais dela.
Hoje, o ministro da Saúde da Noruega
fala dos serviços de saúde para pacientes.
O paciente deve receber aconselhamento
do médico sobre o que fazer.
Isso é um grande progresso,
mas também coloca mais
responsabilidade sobre vocês.
Vocês precisam assumir o comando
junto com seu médico
e decidir juntos qual caminho tomar.
A próxima vez que estiverem
num consultório médico,
quero que vocês perguntem:
"Doutor, isso é mesmo necessário?"
Percebam que, no caso de muitos pacientes,
a resposta seria "não".
Mas também poderia haver
justificativa para cirurgia.
Então, médicos, quais são os riscos
relacionados a essa operação?
Entre 5% e 10% dos pacientes
apresentarão piora na dor.
Entre 1% e 2% dos pacientes
apresentarão infecção na ferida
ou hemorragia, que pode levar
a uma nova cirurgia.
Desses pacientes, 0,5 % apresentará
rouquidão permanente,
e alguns apresentarão redução da função
do braço ou das pernas.
"Doutor, há outras opções?"
Sim, repouso e fisioterapia
durante algum tempo podem
trazer recuperação plena.
"E o que acontece se eu não fizer nada?"
Isso não é recomendado, mas até
nesse caso há uma pequena chance
de recuperação.
Quatro perguntas simples.
Considere isso a sua nova
caixa de ferramentas
para nos ajudar.
"Isso é realmente necessário?"
"Quais são os riscos?"
"Há outras opções?"
"E o que acontece se eu não fizer nada?"
Faça essas perguntas quando seu médico
pedir uma ressonância,
prescrever antibióticos
