Video 18. Imaginal Exposure for OCD: Dr. Hembree and Mia. Wright/APA Publishing

  • 0:05 - 0:07
Dr. Hembree: okay Mia a great
    job on your homework
  • 0:07 - 0:10
    that was really excellent
    you listen to the
  • 0:09 - 0:12
    imaginal exposure recording about six
  • 0:12 - 0:17
times and you did the exposure the
  • 0:17 - 0:18
in vivo exposure of walking around and
  • 0:18 - 0:22
touching things like doorknobs and other
  • 0:21 - 0:23
    surfaces and objects you normally
  • 0:23 - 0:25
wouldn't touch both at your house and
    and
  • 0:25 - 0:28
other places you went and you sat down
  • 0:28 - 0:32
on the floor and you even ate lunch off
    the floor at your
  • 0:32 - 0:33
house and at your mom's house
    Mia: mhmm
  • 0:33 - 0:36
Dr. Hembree: okay and no washing
    Mia: no no no
  • 0:35 - 0:39
hand washing no extra washing perfect
  • 0:39 - 0:43
good for you okay so would you learn
from all that do you think over the
  • 0:42 - 0:44
course of the time you've done that
  • 0:44 - 0:47
Mia: um it just it got easier
    at the beginning it
  • 0:47 - 0:49
was really hard and I feel like my
    stress
  • 0:49 - 0:53
were much higher than at the end um so I
  • 0:53 - 0:55
guess that was what we talked about that
    habituation
  • 0:55 - 0:57
    Dr. Hembree: habituation
    right yeah the
  • 0:57 - 1:01
fact that when you do something like
that that initially makes you anxious
  • 0:59 - 1:03
and you do it repeatedly especially in
  • 1:03 - 1:08
for prolonged periods of time yeah your
anxiety comes down right we're just not
  • 1:08 - 1:13
built to stay indefinitely anxious okay
you notice any other changes or anything
  • 1:13 - 1:18
    else?
Mia: mmm I noticed that at before doing
  • 1:18 - 1:21
    the
exposures I was really really anxious um
  • 1:21 - 1:24
and then it kind of went down like the
  • 1:24 - 1:27
    anticipatory anxiety that would also
    Dr. Hembree: right right
  • 1:27 - 1:30
    Mia: discuss
Dr. Hembree: right did that change by say
  • 1:29 - 1:34
    yesterday
Mia: yeah also the SATs were
  • 1:34 - 1:36
    initially really high before the
    exposure
  • 1:36 - 1:37
and then they went down the
  • 1:37 - 1:39
    more I did that
Dr. Hembree: so you're saying you not
  • 1:39 - 1:42
only noticed that it went down during
the exposure but you started feeling
  • 1:42 - 1:44
    less anxious even approaching
    Mia: um hum yeah
  • 1:44 - 1:47
    Dr. Hembree: okay good
perfect okay all right good okay
  • 1:47 - 1:49
all right so you're doing so well
    we're gonna take another
  • 1:49 - 1:52
step up the hierarchy today like we
    planned
  • 1:52 - 1:56
right okay so this is the dog leash day
    Mia: um hum
  • 1:56 - 1:57
Dr. Hembree: did you bring it
    Mia: yeah
  • 1:57 - 1:59
    Dr. Hembree: okay good
all right we'll get to that later in the
  • 1:59 - 2:02
session first we're gonna do some
  • 2:01 - 2:03
imaginal exposure okay and you remember
  • 2:03 - 2:06
what the purpose of that is why why are we
  • 2:06 - 2:10
    gonna do imaginal?
Mia: um that way I can
  • 2:09 - 2:12
kind of relive a story or a situation
  • 2:12 - 2:15
    that would make me really
    anxious and
  • 2:16 - 2:18
eventually deal with the fear of
  • 2:17 - 2:20
    consequence
    Dr. Hembree: exactly
  • 2:20 - 2:21
    Mia: of getting sick
    in my case
  • 2:21 - 2:25
    Dr. Hembree: right so the
idea is this is how we um are
  • 2:25 - 2:27
able to have you have exposure to or
  • 2:27 - 2:30
sort of a confrontation in imagination
  • 2:29 - 2:33
with the consequences you fear of
  • 2:31 - 2:34
    touching things that are contaminated
  • 2:34 - 2:38
and not washing not doing your rituals
    okay perfect
  • 2:37 - 2:40
so did you write the story?
    Mia: yeah
  • 2:39 - 2:42
    Dr. Hembree:
    okay let me see [coughs]
  • 2:43 - 2:48
all right good all about taking
the dog for a walk with
  • 2:48 - 2:52
that leash and what happens because of
    it excellent
  • 2:52 - 2:55
okay good you know this so I am not
gonna sit here and read it but
  • 2:55 - 2:56
    Mia: uh humm
Dr. Hembree: okay so in a
  • 2:56 - 2:59
minute when we start what I'm gonna
  • 2:59 - 3:02
ask you to do is to close your eyes and
  • 3:02 - 3:08
to sort of narrate or or describe the
scene that you've thought about it this
  • 3:08 - 3:13
week and wrote out where you're handling
the dog leash and very aware of the way
  • 3:13 - 3:18
it's contaminated and I want you to
describe in the story not just what
  • 3:18 - 3:20
you're doing but also what you're kind
  • 3:20 - 3:22
of feeling and what you're thinking as
  • 3:22 - 3:25
you do this and then also leading to the
  • 3:24 - 3:28
consequences you fear from handling the
  • 3:28 - 3:31
dog leash and not washing okay any
  • 3:31 - 3:33
    questions before you start
    Mia: no
  • 3:33 - 3:36
    Dr. Hembree: okay so
    what's your level right now
  • 3:35 - 3:38
Mia: um I would say like a 70
  • 3:38 - 3:41
Dr. Hembree: 70 okay all right go ahead
    and start
  • 3:41 - 3:43
    when you're ready
    Mia: um
  • 3:43 - 3:44
    Dr. Hembree: [inaudible] thank you
  • 3:44 - 3:46
    for reminding me yup
we'll start the recording so you
  • 3:46 - 3:48
    have that for
    homework okay
  • 3:49 - 3:54
Mia: so I'm in my house
and the dog starts barking and I know
  • 3:54 - 3:56
that that means that I have to take him
  • 3:56 - 4:00
    out um I
really don't want to so I'm thinking
  • 4:00 - 4:06
maybe I should wait until Joe gets home
and he can do it but I know the dog
  • 4:05 - 4:09
keeps barking he's gonna wake up the
    baby
  • 4:09 - 4:16
so I decided to go to the front door and
get the leash I'm not gonna wear the
  • 4:16 - 4:25
gloves are usually wear so I fully
    experience um touching the leash
  • 4:25 - 4:28
but I'm already feeling really anxious
  • 4:27 - 4:29
and I look at the leash and I see that
  • 4:29 - 4:34
it's gross and full of hair and the dog
  • 4:34 - 4:37
has been biting on it and it probably
  • 4:37 - 4:40
has saliva and other things and he
  • 4:39 - 4:42
stinks [inaudible] been thinking or if
  • 4:42 - 4:46
that's dog poop and I get sick and I
  • 4:46 - 4:49
just feel myself shaking and really
  • 4:49 - 4:53
sweaty and my heart is beating really
  • 4:52 - 4:56
fast so I'm feeling really really
  • 4:56 - 5:00
anxious but I push myself and I grab the
  • 5:00 - 5:05
leash and I get the dog open the door go
  • 5:05 - 5:09
    out and grab the leash
Dr. Hembree: what's your level now Mia
  • 5:09 - 5:11
    real quick
    Mia: um like 90
  • 5:11 - 5:14
    Dr. Hembree: 90 okay
you're doing great just keep going
  • 5:14 - 5:17
Mia: so I grab the leash and the dog is
  • 5:17 - 5:21
pulling and I feel the leash kind of
  • 5:21 - 5:22
pushing through my skin and I'm thinking
  • 5:22 - 5:25
all the bacteria that's getting there
  • 5:25 - 5:28
and I'm gonna get sick I wanna vomit but
  • 5:27 - 5:31
I just keep going so I walk the dog come
  • 5:31 - 5:36
back home and I just leave the leash in
  • 5:36 - 5:40
the front door and I'm thinking I'm
  • 5:39 - 5:42
probably gonna get sick I really
  • 5:42 - 5:44
    wanna
wash my hands really wanna take a
  • 5:44 - 5:48
shower but I try not to and I stopped
  • 5:48 - 5:52
    myself and I'm I'm
    feeling really really
  • 5:52 - 5:57
anxious so I can't stop thinking about
  • 5:56 - 6:00
washing so I tried to go to bed decide
  • 6:00 - 6:04
to fall asleep the next morning I wake
  • 6:04 - 6:09
up and I already feels like I know I'm
  • 6:08 - 6:11
    sick I'm shivering and
  • 6:10 - 6:14
and I feel like I have a fever and I'm
  • 6:13 - 6:17
shaking again and I'm really nauseated
  • 6:17 - 6:19
so I go to the bathroom start vomiting
  • 6:19 - 6:22
    and I know I'm sick I I already got
  • 6:22 - 6:25
something I should have washed I started
  • 6:25 - 6:28
thinking about that gonna get the baby
  • 6:27 - 6:33
sick and I feel really really sick
    Dr. Hembree: okay
  • 6:33 - 6:36
great job what's your level now
    Mia: like 80
  • 6:36 - 6:38
    Dr. Hembree: 80
okay all right so just to keep
  • 6:38 - 6:41
visualizing this I'm gonna describe it
  • 6:41 - 6:43
now then you just stay with that image
  • 6:43 - 6:45
as much as you can okay so you're in the
  • 6:45 - 6:48
house and the baby's sleeping and you're
  • 6:48 - 6:49
trying to get some housework done and do
  • 6:49 - 6:51
things for dinner suddenly you hear the
  • 6:51 - 6:54
dog at the front door barking and
  • 6:53 - 6:56
barking and immediately your heart rate
  • 6:56 - 6:58
kind of jumps up and you get upset and
  • 6:58 - 7:00
anxious because you know that means he
  • 7:00 - 7:02
needs to go out and Joe is not going to
  • 7:02 - 7:03
be home for hours and you really don't
  • 7:03 - 7:07
wanna take him out but you have to and
  • 7:07 - 7:09
it makes you scared and upset because
  • 7:09 - 7:10
you're not going to use the gloves that
  • 7:10 - 7:12
you always use when you have to take
  • 7:12 - 7:14
him out so you walk toward the door and
  • 7:14 - 7:17
you think well maybe I'll just make em
  • 7:17 - 7:18
wait but you realize he'll wake the baby
  • 7:18 - 7:22
up if you don't take em so you go
  • 7:21 - 7:24
toward the leash and it just makes you
  • 7:23 - 7:26
even more anxious to see it and it's
  • 7:26 - 7:30
dirty and it's chewed up and it's gross
  • 7:29 - 7:32
and you see stains on it and you think
  • 7:32 - 7:33
about what those are from and maybe
  • 7:33 - 7:35
they're from dog poop and that's gonna
  • 7:35 - 7:38
to get on you and you pick it up anyway
  • 7:38 - 7:40
and snap it on his collar and as you go
  • 7:40 - 7:43
out the door and he lunges outside he
  • 7:43 - 7:45
pulls that leash into your hand and you
  • 7:45 - 7:48
feel the contamination from the leash
  • 7:47 - 7:51
entering your body through the pores of
  • 7:51 - 7:52
your skin and you feel like it's just
  • 7:52 - 7:54
running up your arm through your
  • 7:54 - 7:58
bloodstream and you keep thinking oh my
  • 7:58 - 8:02
God what if I get sick from this what if
what if there's some germs in this or
  • 8:02 - 8:08
bacteria but you keep walking him until
he finishes what he has to do and you
  • 8:08 - 8:10
bring him back inside the house take the
  • 8:10 - 8:14
leash off and the urge to wash is so
  • 8:14 - 8:16
powerful so strong you want to go scrub
  • 8:16 - 8:20
    your hands and
get in the shower and wash your whole
  • 8:19 - 8:24
body and do it quickly and get all these
germs off from the leash but you don't
  • 8:24 - 8:30
do that you don't you make yourself not
do the ritual and then you go back in
  • 8:29 - 8:33
then just keep doing the housework and
keep doing things but that's the sense
  • 8:33 - 8:38
of feeling sick is staying with you and
worrying about what's going to happen if
  • 8:38 - 8:45
you picked up germs from the leash that
night you go to sleep still anxious
  • 8:45 - 8:48
still upset about what happened wake up
  • 8:47 - 8:51
in the morning and then before you have
  • 8:50 - 8:53
your eyes open even a couple minutes you
  • 8:52 - 8:54
know that you're sick you can feel your
  • 8:54 - 8:58
body being sick you rush to the bathroom
  • 8:57 - 9:02
and vomit you can feel the fever and
  • 9:01 - 9:04
it's all in your body is just not not
  • 9:04 - 9:05
right you're sick and you know it's
  • 9:05 - 9:07
because you handled that leash and
  • 9:07 - 9:10
didn't wash you didn't do the washing
  • 9:10 - 9:13
you needed to do and now you're sick and
  • 9:12 - 9:15
the baby will probably get sick and if
  • 9:15 - 9:18
only you had done the washing okay
  • 9:18 - 9:23
    what's your level now
    Mia: like eighty
  • 9:23 - 9:26
    Dr. Hembree: 80 okay
okay you take it back and you do it
  • 9:26 - 9:33
    again okay good job
Mia: um okay so I'm home um the
  • 9:33 - 9:36
    dog starts barking
